深层巩膜切除联合新鲜羊膜植入术对青光眼术后滤过功能的影响

摘　　要：目的探讨深层巩膜切除联合新鲜羊膜移植术对青光眼滤过功能的影响。方法青光眼患者90例(106只眼),按数字表法随机分为治疗组45例(53只眼)和对照组45例(53只眼),对照组行小梁切除术,治疗组在此基础上加深层巩膜切除、新鲜羊膜移植。观察术前、术后1 m、3 m、6 m的视力、眼压及滤过泡情况。结果(1)最佳矫正视力:①术后1 m、3 m、6 m与术前对比,差异均有统计学意义(P<0.05),②术后1 m,两组比较无统计学意义(t=0.227,P>0v05);术后3 m、6 m两组比较差异有统计学意义(t值分别为2.256、2.273,P<0.05)。(2)眼压:①术后1 w、1 m、3 m、6 m与术前比较,两组差异均有统计学意义(P<0.05),②术后1 w、3 m、6 m两组差异具有统计学意义(t值分别为3.061、3.578、4.063,P<0.05);术后1 m,两组差异无统计学意义(t=0.437,P>0.05)。(3)滤过泡:术后1m,两组差异无统计学意义(χ^(2)=3.757,P=0.057);术后3 m、6 m,两组差异有统计学意义(2分别为3.975、4.711,P<0.05)。(4)手术总有效率:术后6个月,两组比较差异有统计学意义(χ^(2)=7.661,P=0.008)。结论深层巩膜切除联合新鲜羊膜移植术治疗青光眼,术后滤过泡滤过功能更加稳定,成为青光眼新的术式之一。Objecitve To explore the effect of deep sclera resection combined with fresh amniotic membrane transplantation on the filtration function of glaucoma.Methods 90 glaucoma patients(106 eyes)were selected,and 45 cases were randomly assigned to the treatment group and the control group using the number table method.The control group underwent trabeculectomy,and the treatment group received deep sclerectomy and fresh amniotic membrane on this basis.transplant.Observe the visual acuity,intraocular pressure and filtering bleb of 1m,3m,6m before and after operation.Results(1)Best corrected visual acuity:①Compared with preoperative 1m,3m,6m after operation,the difference was statistically significant(P<0.05),②1m after operation,there was no statistically significant difference between the two groups(t=0.227,P>0.05);the difference between the two groups at 3m and 6m after surgery was statistically significant(t values were 2.256,2.273,P<0.05).(2)Intraocular pressure:①Compared with preoperative 1w,1m,3m,6m after operation,the difference between the two groups was statistically significant(P<0.05),②The difference between the two groups was statistically significant at 1w,3m,and 6m after operation(T values were 3.061,3.578,4.063,P<0.05);1m after operation,the difference between the two groups was not statistically significant(t=0.437,P>0.05).(3)Filtering bleb:1m after surgery,the difference between the two groups was not statistically significant(χ^(2)=3.757,P=0.57);3m and 6m after surgery,the difference between the two groups was statistically significant(2 was 3.975,4.711 respectively,P<0.05).(4)The total effective rate of the operation:6 months after the operation,the difference between the two groups was statistically significant(χ^(2)=7.661,P=0.008).Conclusion Deep sclerectomy combined with fresh amniotic membrane transplantation for the treatment of glaucoma,the postoperative filtering function of the filtering bleb will be more stable,becoming one of the new surgical procedures for glaucoma.
