调脂理肝汤联合穴位埋线治疗非酒精性脂肪性肝炎痰湿瘀阻证临床研究

摘　　要：目的:观察调脂理肝汤及调脂理肝汤联合穴位埋线治疗非酒精性脂肪性肝炎的痰湿瘀阻证型的疗效。方法:非酒精性脂肪性肝炎中医辨证属于痰湿瘀阻证型患者180例,随机分成治疗组A、治疗组B和对照组。治疗组A治疗方案为调脂理肝汤(150ml,tid),治疗组B治疗方案为调脂理肝汤(150ml,tid)联合穴位埋线治疗(1次/2周);对照组治疗方案为口服水飞蓟宾葡甲胺片(湖南千金协力药业有限公司)150mg,tid。观察时间12周。观察治疗前后临床症状的改善及肝功能、B超、肝脏弹性指数等的变化。结果:疗程结束时,治疗组(A、B组)与对照组相比临床症状、体征和肝功能、影像学指标改善情况均有明显差异,治疗组疗效优于对照组(P<0.05),治疗组A组与B组疗效对比无明显差异,但治疗B组对体重的控制优于治疗A组,体重下降有显著差异(P<0.05)。结论:调脂理肝汤及调脂理肝汤联合穴位埋线治疗非酒精性脂肪性肝炎的痰湿瘀阻证型具有良好的改善症状、保肝降脂及改善肝脏形态作用且无明显的毒副反应。Objective:To observe the curative effect of using Tiaozhi Ligan Decoction and Tiaozhi Ligan Decoction combined with acupoint catgut embedding in treating non-alcoholic steatohepatitis with phlegm dampness and blood stasis syndrome.Methods:180 patients with TCM syndrome differentiation of non-alcoholic steatohepatitis were randomly divided into a treatment group A,a treatment group B and a controlled group.The treatment plan of the treatment group A was Tiaozhi Ligan Decoction 150ml tid,while the treatment plan of the treatment group B was Tiaozhi Ligan Decoction 150ml tid unite acupoint catgut embedding therapy 1 time/2 weeks.The controlled group received oral Silybin Glumethylamine Tablets(Hu'nan Qianjin Xiexi Pharmaceutical Co.LTD.)150mg tid.The observation period was 12 weeks.The improvement of clinical symptoms and the changes of liver function,B-ultrasound and liveRelasticity index before and afteRtreatment were observed.Results:At the end of the course of treatment,there were significant differences in clinical symptoms,liveRfunction and imaging indicators between the treatment group(the group A and the group B)and the controlled group.The curative effect of the treatment group was betteRthan that of the controlled group(P<0.05).There was no significant difference between the treatment group A and the treatment group B.However,the weight control of the group B was betteRthan that of the group A,and there was significant difference in weight loss(P<0.05).Conclusion:Using Tiaozhi Ligan Decoction and Tiaozhi Ligan Decoction combined with acupoint injection in the treatment of non-alcoholic steatohepatitis with phlegm,dampness and blood stasis syndrome have good effects on improving symptoms,protecting liveRand lowering lipid and improving liveRmorphology without obvious toxic and side effects.
