腹腔镜辅助小切口胃癌根治术治疗远端胃癌的疗效及对血清肿瘤标志物的影响

摘　　要：目的探讨腹腔镜辅助小切口胃癌根治术治疗远端胃癌的疗效及对血清肿瘤标志物的影响。方法将150例行远端胃癌根治术的胃癌患者按照手术方式不同分为两组,观察组(n=76)采用腹腔镜辅助小切口远端胃癌根治术治疗,对照组(n=74)采用传统开腹手术治疗,比较两组患者手术时间、淋巴结清扫数目、术中出血量、术后胃肠道功能恢复时间、术后开始进食时间、住院时间、并发症发生率及癌胚抗原(CEA)、糖类抗原19-9(CA19-9)、神经元特异性烯醇化酶(NSE)水平。结果观察组患者手术时间、术后胃肠功能恢复时间、术后开始进食时间、住院时间均短于对照组,术中出血量少于对照组,术后CEA、CA19-9、NSE水平和并发症发生率均低于对照组,差异均有统计学意义(P﹤0.05)。结论腹腔镜辅助小切口远端胃癌根治术能提高患者治疗效果并不增加并发症的发生,降低患者血清肿瘤标志物水平。Objective To investigate the curative effect of laparoscopic-assisted small incision radical gastrectomy for distal gastric cancer and its effect on serum tumor markers.Method A total of 150 patients with distal gastric cancer undergoing radical gastrectomy were divided into the observation group(n=76,treated with laparoscopic-assisted radical gastrectomy)and the control group(n=74,treated with traditional open surgery).The operation time,number of lymph node dissection,intraoperative blood loss,postoperative gastrointestinal function recovery time,postoperative feeding time,hospitalization time,complication rate,carcinoembryonic antigen(CEA),carbohydrate antigen 19-9(CA19-9)and neuron specific enolase(NSE)were compared between the two groups.Result The operation time,postoperative gastro-intestinal function recovery time,postoperative feeding time,hospitalization time,intraoperative blood loss,postoperative CEA,CA19-9,NSE and complication rate of the observation group were all lower than those of the control group(P<0.05).Conclusion Laparoscopic-assisted small incision radical gastrectomy for distal gastric cancer can improve the cu-rative effect of patients without increasing the complications,and can reduce the serum tumor markers.
