摘  要:目的:生物力学比较开放与闭合愈合环境对即刻负载的正畸用不锈钢微螺旋种植体的影响。方法:48枚不锈钢正畸微种植体(型号SS1312-07),植入于2头18个月龄的微型猪上下颌骨内。随机分为两组:闭合愈合组及开放愈合组。闭合愈合组微种植体的植入是在植入点切开粘骨膜,暴露骨面,将微种植体用自攻种植法植入,即刻测量最高植入转矩。在相近的两个种植体用闭合镍钛弹簧加载约150g水平力,缝合创口。开放愈合组微种植体的植入不切开粘骨膜,植入时直接穿透粘骨膜自攻植入,测量最高植入转矩,加载。负载10周后检查微种植体的稳定性,并测量最高旋出转矩。配对t检验比较两组微种植体的生物力学差异,有统计学意义(P<0.05)。Pearson检验评价植入转矩与去除转矩的相关关系。结果:(1)植入时共有4枚不锈钢微种植体折断,折断率8%;(2)10周后临床成功率:闭合愈合组有3枚种植体松动,成功率87.5%,开放愈合组有4枚种植体松动,成功率为75.0%(P<0.05);(3)最高植入转矩比较:开放愈合组(6.59±1.03)N cm,闭合愈合组(6.51±1.10)N cm,差异无统计学意义(P>0.05);去除转矩比较:闭合愈合组(3.01±1.54)N cm,开放愈合组(2.45±1.17)N cm,差异有统计学意义(P<0.05)。开放愈合环境的最高植入转矩和最高去除转矩均呈有相关性(r=0.601),闭合愈合环境的最高植入转矩和最高去除转矩均呈有相关性(r=0.642,P<0.05)。结论:不锈钢正畸微种植体用作正畸支抗,建议有一个闭合环境的愈合期。Objective:The biomechanical evaluation of the effects of the open and closed healing environment to orthodon-tic stainless steel mini-implants.Methods:48 stainless steel mini-implants(SS1312-07)were implanted into the jaws of two adult mini-pigs,aged 18ms.They were random divided into two groups:closed healing group and open healing group.In the closed healing group,the mini-implants were inserted after a incisor and jaw bone was exposed.In the open healing group,the mini-implants were inserted without any incisor.The peak insertion torque was measured immediately after implantation.The ap-proximately 150g level force was loaded immediately to all mini-implants.The peak removal torque was measured after 10-weeks loading.The correlation between the peak insertion torque and the peak removal torque was analyzed by paired t-test.P<0.05 means statistically significant difference.Results:(1)four stainless steel micro implants were broken at the time of im-plantation,with a fracture rate of 8%;(2)The clinical success rate after 10 weeks:there were 3 implants loose in the closed heal-ing group,the success rate was 87.5%,and 4 implants in the open healing group were loose,the success rate was 75.0%(P<0.05);(3)Comparison of the highest implantation torque:open healing group(6.59±1.03)N cm,closed healing group(6.51±1.10)N cm.The difference was not statistically significant(P>0.05);Removal torque comparison:closed healing group(3.01±1.54)N cm,open healing group(2.45±1.17)N cm,the difference was statistically significant(P<0.05).There was a correla-tion between the highest implantation torque and the highest removal torque in open healing environment(r=0.601),and there was a correlation between the highest implantation torque and the highest removal torque in closed healing environment(r=0.642,P<0.05).Conclusion:The stainless steel mini-implants served as orthodontic anchorage were suggested to have in closed environment healing period.
