非酒精性脂肪性肝病患者血清趋化素水平变化及其与胰岛素抵抗、炎症的关系研究

摘　　要：目的探讨非酒精性脂肪性肝病(Nonalcoholic fatty liver disease,NAFLD)患者血清趋化素(Chemerin)水平变化及其与胰岛素抵抗、炎症的相关性。方法选取NAFLD患者60例,按照超声影像肝脏损伤程度分为三个亚组,即轻度组(B组)、中度组(C组)、重度组(D组),正常健康体检者22例为健康对照组(A组),测量各组患者身高、体重、血压并计算体质指数;检测并比较各组间甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、丙氨酸氨基转移酶(ALT)、空腹血糖(FBG)、血清胰岛素(FINS)、Chemerin、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)、胰岛素抵抗指数(HOMA-IR)等指标。结果与健康对照组比较,NAFLD三个亚组血清Chemerin、HOMA-IR、hs-CRP、TNF-α水平升高(P<0.05);D组Chemerin、HOMA-IR、hs-CRP、TNF-α水平明显高于C组与B组,差异具有统计学意义(P<0.05);相关分析显示,血清Chemerin水平与BMI、TG、FBG、HOMA-IR、hs-CRP、TNF-α呈正相关(r分别为0.699、0.454、0.409、0.434、0.589、0.561,P<0.05),与HDL-C呈负相关(r=-0.357,P<0.05)。结论NAFLD患者血清Chemerin呈高水平表达状态,血清Chemerin水平与HOMA-IR、hs-CRP、TNF-α水平相关。Objective To investigate the changes of serum Chemerin level in patients with nonalcoholic fatty liver disease(NAFLD)and its relationship with insulin resistance and inflammation.Methods 60 patients with NAFLD were selected and divided into mild group(group B),moderate group(group C),severe group(group D)according to the degree of liver injury on ultrasound images.22 healthy perpons were selected as healthy control group(group A).Height,weight,blood pressure and body mass index were measured.TG,TC,HDL-C,LDL-C,ALT,FBG,FINS,Chemerin,hs-CRP,TNF-αand HOMA-IR were detected and compared among each group.Results Compared with group A,the levels of serum Chemerin,HOMA-IR,hs-CRP and TNF-αin three subgroups of NAFLD were increased(P<0.05).The levels of Chemerin,HOMA-IR,hs-CRP and TNF-αin group D were significantly higher than those in group C and group B(P<0.05).Correlation analysis showed that Chemerin level was positively correlated with BMI,TG,FBG,HOMA-IR,hs-CRP and TNF-α(r=0.699,0.454,0.409,0.434,0.589,0.561,P<0.05),and negatively correlated with HDL-C(r=-0.357,P<0.05).Conclusion The expression of serum Chemerin is high in patients with NAFLD.The serum Chemerin level is correlated with HOMA-IR,hs-CRP and TNF-α.
