乳腺癌淋巴结转移误诊为蜂窝织炎1例

摘　　要：近年来,蜂窝织炎疾病有较高的发病率,是急诊科常见病种。本报道中该患者以反复颜面水肿为主要表现就诊,初步诊断为颌面部蜂窝织炎,发病后经长期抗感染治疗,病情反复,后经多学科会诊、影像学检查及穿刺活检,最终确诊为乳腺癌淋巴结转移,后转至当地医院治疗。此患者早期未能及时诊断,且相关病例报道较为少见,应充分吸取经验,认识到引起颜面水肿的原因较多,对伴有腋窝淋巴结肿胀的患者应想到乳腺癌淋巴转移的可能,及早进行穿刺确诊。对迁延不愈的患者在入院后详细询问病史和全面查体,发现异常,拓宽诊疗思路。本文总结相关诊治经验,并复习参考文献,供临床医师参考。In recent years,cellulitis has a higher incidence of disease and it is a common disease in the emergency department.This report in the patients with facial edema repeatedly as the main performance,the preliminary diagnosis of maxillofacial cellulitis,after the onset of the anti-infection treatment for a long time,the illness repeatedly,after multidisciplinary consultation,imaging examination and biopsy,eventually diagnosed with breast cancer lymph node metastasis,he was transferred to a local hospital for treatment.The early diagnosis of this patient was not timely,and related case reports were rare,so the experience should be fully learned,and the causes of facial edema should be recognized.For patients with axillary lymph node swelling,the possibility of breast cancer lymphatic metastasis should be considered,and early diagnosis should be made by puncture.For patients with delayed recovery,detailed medical history and comprehensive physical examination were conducted after admission to find abnormalities and broaden the diagnosis and treatment ideas.This article summarizes the relevant diagnosis and treatment experience,and reviews the references for clinicians’reference.
