摘 要:目的探讨低剂量和常规剂量螺旋CT扫描在肺癌早期筛查中的应用价值。方法从2018年11月至2019年9月间在本院健康体检者中选择符合肺癌高危人群标准的受试者284例。将其中进行低剂量螺旋CT扫描的214例纳入研究组,进行常规剂量螺旋CT扫描的70例纳入对照组。评估两组图像质量,记录肺结节、陈旧性肺结核、炎性病灶等病灶筛查情况。结果研究组图形质量为良好的患者比例明显低于对照组,一般的患者比例明显高于对照组,差异均有统计学意义(均P<0.05)。肺结节、陈旧性肺结核、炎性病灶、肺大泡/肺气肿、纵膈结节的检出率及阴性率在两组间比较差异均无统计学意义(均P>0.05)。研究组平均辐射CT剂量(0.81±0.15)mSv、平均剂量长度乘积(49.78±7.47)mGy/cm,均明显低于对照组,差异均有统计学意义(均P<0.01)。结论低剂量螺旋CT在减少辐射剂量的同时仍能够保证较好的扫描图像质量,不影响肺部病灶的检出率,具有较高的应用价值。Objective To explore the application value of low-dose and conventional dose spiral CT in early screening of lung cancer.Methods From November 2018 to September 2019,284 subjects who met the criteria of high-risk group of lung cancer were selected.214 cases of low-dose spiral CT scanning were included in the study group,and 70 cases of conventional dose spiral CT scanning were included in the control group.The image quality was evaluated and the screening of pulmonary nodules,old pulmonary tuberculosis and inflammatory lesions was recorded.Results The proportion of patients with good image scanning quality in the study group was significantly lower than that in the control group,and the proportion of general patients was significantly higher than that in the control group,the difference was statistically significant(P<0.05).There was no significant difference in the proportion of patients with poor image scanning quality between the two groups(P>0.05);there was no significant difference in the detection rate of nodules between the two groups(P>0.05).There was no significant difference in the detection rate and negative rate of pulmonary nodules,old pulmonary tuberculosis,inflammatory lesions,bullae/emphysema,mediastinal nodules between the two groups(P>0.05).The average radiation CT dose(0.81±0.15)mSv and average dose length product(49.78±7.47)mGy/cm in the study group were significantly lower than those in the control group(P<0.01).Conclusion Low dose spiral CT can reduce radiation dose and ensure good image quality without affecting the detection rate of lung lesions,so it has high application value.
