摘 要:目的了解温州地区体检人群胃蛋白酶原、胃泌素-17水平的动态变化及与性别、年龄的关系,为胃癌的早期筛查提供参考依据。方法选择2018年5月—2019年12月在温州医科大学附属第一医院进行体检的人员3746例,测定其胃蛋白酶原I(PGⅠ)、胃蛋白酶原Ⅱ(PGⅡ)及胃泌素-17(G17),计算PGⅠ/PGⅡ比值,根据性别、年龄、月份进行分组,利用统计软件进行相关分析。结果Mann-Whitney U检验发现,男性体检人群PGⅠ、PGⅡ、PGⅠ/PGⅡ水平均高于女性体检人群,男性体检人群G17水平明显低于女性体检人群,差异有统计学意义(P<0.001)。Spearman相关系数分析显示,G17(r=0.115)、PGⅠ(r=0.239)、PGⅡ(r=0.280)与年龄呈正相关,随年龄的增加呈上升趋势(P<0.05);PGⅠ/PGⅡ(r=-0.115)与年龄呈负相关,随着年龄的增加呈下降趋势(P<0.05)。经Kruskal-Wallis H检验发现,6个年龄段组的不同月份间PGⅠ和PGⅠ/PGⅡ水平差异均有统计学意义(P<0.01)。结论随着年龄的增加,温州地区体检人群血清PGⅠ、PGⅡ、G17呈现上升趋势,PGⅠ/PGⅡ呈现下降趋势,且还受性别的影响,临床上对PGⅠ、PGⅡ、PGⅠ/PGⅡ、G17进行分析时应考虑年龄、性别等因素的影响。Objective To understand the dynamic changes of pepsinogen and gastrin-17 levels in population for physical examination and their relationship with gender and age in Wenzhou area,so as to provide reference for the early screening of gastric cancer.Methods From May 2018 to December 2019,a total of 3746 people who had physical examination in the First Affiliated Hospital of Wenzhou Medical University were selected to measure their pepsinogen I(PGⅠ),pepsinogen II(PGⅡ)and gastrin-17(G17),and calculate the PGⅠ/PGⅡratio.They were divided into groups according to gender,age and month,and analyzed by statistical software.Results It was found by Mann Whitney U test that the levels of PGⅠ,PGⅡand PGⅠ/PGⅡin male physical examination group were higher than that in female physical examination group,and the level of G17 in male physical examination group was significantly lower than that in female physical examination group(P<0.001).According to Spearman correlation coefficient analysis,G17(r=0.115),PGⅠ(r=0.239),PGⅡ(r=0.280)were positively correlated with age,and increased with age(P<0.01);PGⅠ/PGⅡ(r=-0.115)was negatively correlated with age,and decreased with age(P<0.05).Kruskal Wallis H test showed that there were significant differences in PGⅠand PGⅠ/PGⅡlevels among the six age groups in different months(P<0.01).Conclusion With the increase of age,the serum PGⅠ,PGⅡand G17 levels in the population for physical examination showed an upward trend,and the PGⅠ/PGⅡshowed a downward trend,which was also affected by gender.Therefore,the influence of age,gender and other factors should be considered in the clinical analysis of PGⅠ,PGⅡ,PGⅠ/PGⅡand G17.
