盐酸多奈哌齐联合尼莫地平治疗血管性痴呆的效果观察

摘　　要：目的:分析盐酸多奈哌齐联合尼莫地平治疗血管性痴呆的临床效果。方法:回顾性纳入河南中医药大学第三附属医院2015年12月至2019年12月收治的132例血管性痴呆患者。根据用药方案将纳入患者分为盐酸多奈哌齐组(62例)和联合组(70例)。比较两组的治疗效果,治疗前后痴呆程度、认知功能及日常生活能力和不良反应发生情况。结果:治疗3个月后,盐酸多奈哌齐组治疗总有效率为77.4%(48/62),低于联合组的94.3%(66/70),差异有统计学意义(χ2=7.942,P=0.005)。治疗3个月后,盐酸多奈哌齐组痴呆评定量表(CDR)评分高于联合组,智力状态检查量表评分和日常生活能力量表评分低于联合组,差异有统计学意义(P<0.05)。盐酸多奈哌齐组不良反应发生率(14.5%,9/62)与联合组(14.3%,10/70)比较差异未见统计学意义(P>0.05)。结论:盐酸多奈哌齐联合尼莫地平治疗血管性痴呆患者疗效较好,能有效降低痴呆患者的痴呆程度,提高患者的认知能力和日常生活能力,且不良反应发生率低,安全性高。Objective To analyze the effects of donepezil hydrochloride combined with nimodipine on patients with vascular dementia.Methods A total of 132 patients with vascular dementia admitted to the First Affiliated Hospital of Zhengzhou University from December 2015 to December 2019 were included retrospectively.They were divided into donepezil hydrochloride group(62 cases)and combined group(70 cases)according to medication schemes.The curative effect,preoperative and postoperative dementia degree,cognitive function,activities of daily living,and adverse reactions were compared between the two groups.Results After 3 months of treatment,the total effective rate of donepezil hydrochloride group was 77.4%(48/62),significantly lower than the 94.3%(66/70)of combined group(χ2=7.942,P=0.005).After 3 months of treatment,clinical dementia rating score in donepezil hydrochloride group were significantly higher than that in combined group,and scores of mini-mental state examimation and activity of daily life were significantly lower than those in combined group(P<0.05).There was no significant difference in incidence of adverse reactions between donepezil hydrochloride group(14.5%,9/62)and combined group(14.3%,10/70),P>0.05.Conclusions Donepezil hydrochloride combined with nimodipine is effective in the treatment of patients with vascular dementia,which can effectively reduce the degree of dementia,significantly improve the cognitive ability and daily living ability of patients with dementia,with low incidence of adverse reactions and high safety.
