摘  要:目的:探讨骨形态生成蛋白2(BMP2)、BMP4的基因多态性与难治性根尖周炎(RAP)风险的相关性。方法:将我院2018年6月至2020年8月期间62例根尖性牙周炎病变治疗失败患者作为RAP组,选择同期根尖性牙周炎病变治疗后根部充实且根尖周围健康的98例作为对照组,比较两组一般资料、通过实时聚合酶链反应技术分析BMP4、BMP2基因位点,探讨与RAP发生的相关性。结果:RAP组BMP2基因rs235768位点TT基因型45.16%及频率63.71%均较对照组21.43%、57.14%高;BMP4基因rs17563位点C基因型74.19%及频率66.13%较对照组48.98%、37.24%高(均P<0.05)。Logistic显示,BMP2基因rs235768基因位点TT基因型(OR=3.370,95%CI1.325~8.574)和BMP4基因rs17563位点CT基因型及TT基因型(OR=0.319,95%CI0.132~0.769;OR=0.196,95%CI0.087~0.444)分别是RAP发生的保护因素和危险因素。结论:BMP2基因rs235768基因位点TT基因型为RAP危险因素,BMP4基因rs17563位点CT基因型及TT基因型是RAP保护因素。Objective:To explore the relationship between the genetic polymorphisms of bone morphogenetic protein 2(BMP2)and BMP4 and the risk of refractory apical periodontitis(RAP).Methods:From June 2018 to August 2020 in our hospital,62 patients with apical periodontitis lesions were treated as the RAP group,and 98 patients with apical periodontitis lesions treatment during the same period were selected with solid roots and healthy apical circumference as the control group,We compared the two groups of general data and analyzed the BMP4 and BMP2 gene loci by real-time polymerase chain reaction technology,then explored the correlation with the occurrence of RAP.Results:The BMP2 gene rs235768 locus TT genotype(45.16%)and frequency(63.71%)in the AP group were higher than those of the control group by 21.43%and 57.14%respectively;the BMP4 gene rs17563 locus C genotype(74.19%)and frequency(66.13%)were higher than those of the control group by 48.98%and 37.24%respectively(P<0.05).Logistic analysis showed that BMP2 gene rs235768 locus TT genotype(OR=3.370,95%CI1.325~8.574)and BMP4 gene rs17563 locus CT genotype and TT genotype(OR=0.319,95%CI0.132~0.769;OR=0.196,95%CI0.087~0.444)were the protective factors and risk factors for the occurrence of RAP.Conclusion:The TT genotype at the rs235768 locus of the BMP2 gene is a risk factor for RAP,and the CT genotype and the TT genotype at the rs17563 locus of the BMP4 gene are the protective factors for RAP.
