老年人根尖周炎常规根管治疗术治疗依从性及影响因素分析

摘　　要：目的:对拟行根管治疗的老年根尖周炎患者进行常规根管治疗依从性调查,并分析其中的影响因素。方法:选取2018年1月~2020年7月在我院口腔科拟行根管治疗的老年根尖周炎患者98例为研究对象。对患者进行问卷调查,收集患者年龄、性别、学历、婚姻状况、工作状态、经济收入(包括退休金)、医疗付费方式、治疗过程关心的问题(疼痛、医生技术水平、疗程、疗效、费用、服务等)及感受(首诊后自觉症状缓解情况、疼痛恐惧程度、对医疗服务态度满意程度、对治疗技术信任程度、认为根管治疗程序复杂与否)、根管治疗认知程度等资料。对可能的影响因素进行单因素非条件Logistic回归分析,再进行非条件Logistic多因素回归分析。结果:在98例老年患者中,问卷有效并纳入分析的有91例,其中依从性好有54例,占59.34%;依从性差有37例,占40.66%。年龄、月收入、医疗付费方式、疼痛恐惧程度、对医疗服务态度满意程度、对治疗技术信任程度、认为根管治疗程序复杂与否、根管治疗认知程度、VAS评分为根尖周炎根管治疗依从性的影响因素(P<0.05)。结论:在根尖周炎常规根管治疗过程中,有相当部分老年患者治疗依从性不佳,需引起重视,其中年龄、月收入、医疗付费方式、疼痛恐惧程度、VAS评分、对医疗服务态度满意程度、对治疗技术信任程度、认为根管治疗程序复杂与否、根管治疗认知程度是对根尖周炎根管治疗的影响因素。口腔医生可根据老年患者的实际情况结合上诉影响因素来制定适合的治疗方案,以提高老年患者的治疗依从性。Objective:The compliance of routine root canal therapy was investigated in elderly patients with periapical periodontitis who planned to receive to root canal therapy,and the influencing factors were analyzed.Methods:A total of 98 elderly patients with periapical periodontitis who were scheduled to undergo root canal treatment in the Department of Stomatology of our hospital from January 2018 to July 2020 were selected as the research objects.Questionnaire survey was conducted on patients to collect patients'age,sex,education,marital status,working status,economic income(including pension),medical payment method,and questions concerned about the treatment process.Pain,doctor's skill level,course of treatment,curative effect,cost,service,etc.And feelings including symptom relief after the first diagnosis,pain fear degree,satisfaction degree of medical service attitude,trust degree of treatment technology,whether the procedure of root canal treatment is complex or not,cognition degree of root canal treatment,etc.The single factor unconditional Logistic regression analysis was performed on the possible influencing factors,and then the unconditional Logistic multiple factor regression analysis was performed.Results:Among 98 elderly patients,the questionnaire was valid and 91 cases were included in the analysis,of which 54 cases had good compliance,accounting for 59.34%;37 cases had poor compliance,accounting for 40.66%.Age,monthly income,medical payment method,degree of pain fear,degree of satisfaction with medical service attitude,degree of trust in treatment technology,whether the root canal treatment procedure is complicated or not,the degree of awareness of root canal treatment,and VAS score for root canal treatment of apical periodontitis Influencing factors of compliance(P<0.05).Conclusion:In the process of conventional root canal treatment for periapical periodontitis,a considerable number of elderly patients have poor treatment compliance,which requires attention,including age,monthly income,medical payment m
