摘　　要：目的:探讨原发性中枢神经系统淋巴瘤(PCNSL)患者的预后影响因素。方法:基于美国国立癌症研究所的监测、流行病学、结果数据库(SEER)选择2004至2015年确诊为PCNSL的患者,采用COX比例风险模型进行单因素和多因素分析确定PCNSL患者的独立预后因素,Kaplan-Meier法绘制生存曲线,Log-rank检验比较不同组别患者的生存率差异。结果:单因素分析显示发病年龄、婚姻状态、原发部位、病理类型、B症状、HIV感染情况、治疗方式与原发性中枢神经系统淋巴瘤患者生存预后密切相关,多因素分析显示发病年龄、婚姻状态、原发部位、病理类型、HIV感染情况、治疗方式是影响PCNSL患者生存预后的独立因素。结论:发病年龄、婚姻状态、原发部位、病理类型、HIV感染情况、治疗方式是影响原发性中枢神经系统淋巴瘤患者生存预后的独立因素。Objective:To investigate the factors that affect the prognosis of primary central nervous system lymphoma(PCNSL).Methods:Patients diagnosed with PCNSL from 2004 to 2015 were screened from the National Cancer Institute’s Surveillance,Epidemiology,and End Results(SEER)database.Univariate and multivariate analyses were performed using the COX proportional hazards model to identify prognostic factors of PCNSL.Survival curves were drawn using the Kaplan-Meier method,and the difference was compared using the Log-rank test.Results:Univariate analysis showed statistically significant associations between overall survival and age at diagnosis,marital status,primary site,histologic subtype,B symptoms,HIV infection and treatment modality(P<0.05).The results of multivariate survival analyses revealed that age at diagnosis,marital status,primary site,histologic subtype,HIV infection and treatment modality are independent prognostic factors of PCNSL.Conclusion:Age at diagnosis,gender,marital status,primary site,histologic subtype,HIV status and treatment modality are independent prognostic factors of PCNSL.
