窄带成像联合放大胃镜技术对提高早期胃癌及癌前病变筛查诊断价值临床研究

摘　　要：目的探讨窄带成像联合放大胃镜技术在早期胃癌及癌前病变筛查中的诊断价值。方法本研究以我院于2018年12月至2020年12月收治的200例胃黏膜病变患者作为研究对象,在本院所选的胃黏膜病变患者全部接受WLE和ME-NBI检查,检查早期胃癌及癌前病变。观察分析NBI放大胃镜联合靛胭脂染色和传统内镜两种诊断方式两种检查方式图像质量评分,比较其检查结果,以病理检查结果为金标准,比较诊断效能。结果WLE检查的微血管及腺管图像质量评分均低于ME-NBI检查的微血管及腺管图像质量评分,差异均有统计学意义(P<0.05)。WLE检查的敏感度、特异度和准确度均低于ME-NBI检查方式,差异具有统计学意义(P<0.05)。在胃癌高危以及非高危人群的胃癌检出率中,高危人群的胃癌检出率较高(P<0.05);且放大胃镜联合靛胭脂染色的检出率要高于传统内镜(P<0.05)。结论窄带成像联合放大胃镜技术在早期胃癌及癌前病变筛查中的诊断价值分析,具有一定临床价值,值得在临床上加以广泛推广以及应用。Objective To explore the diagnostic value of narrowband imaging combined with magnifying gastroscopy in the screening of early gastric cancer and precancerous lesions.Methods In this study,200 patients with gastric mucosal lesions admitted to our hospital from December 2018 to December 2020 were used as the research object.All patients with gastric mucosal lesions selected in this hospital received WLE and ME-NBI examinations.Gastric cancer and precancerous lesions.Observe and analyze the image quality scores of the two diagnostic methods of NBI magnifying gastroscope combined with indigo carmine staining and traditional endoscopy,and compare the inspection results.The pathological results are the gold standard to compare the diagnostic efficiency.Results The image quality scores of microvessels and ducts in WLE examination were lower than those of ME-NBI,and the differences were statistically significant(P<0.05).The sensitivity,specificity and accuracy of WLE inspection are lower than those of ME-NBI inspection.The difference was statistically significant(P<0.05).Among the high-risk and non-high-risk groups of gastric cancer,the detection rate of gastric cancer in high-risk groups is higher(P<0.05);and the detection rate of magnifying gastroscope combined with indigo carmine staining was higher than that of traditional endoscopy(P<0.05).Conclusion The diagnostic value analysis of narrowband imaging combined with magnifying gastroscopy in the screening of early gastric cancer and precancerous lesions has certain clinical value,and it is worthy of being widely promoted and applied in clinic.
