摘  要:目的利用DTI(diffusion tensor imaging,DTI)技术检测无认知障碍2型糖尿病患者脑白质纤维束的微观改变。方法2型糖尿病患者(Type 2 diabetes mellitus,T2DM)20例为T2DM组,年龄、性别相匹配的健康对照组(healthy controls,HCs)20例。T2DM组及对照组均由专科医师行简易精神状态量表(mini mental state examination,MMSE)评分。MRI检查前,行空腹血糖及糖化血红蛋白实验室检查;MRI检查序列包括:DTI、常规MRI序列检查(T1 Flair、T2WI、T2 Flair、DWI),以除外脑血管病、肿瘤等器质性病变。MRI检查结束后,应用GE-ADW4.6工作站将DTI序列进行后处理,得到感兴趣区ROI值(region of interest,ROI)、FA值(fractional anisotropy,FA)、MD值(mean diffusivity,MD)及RD值(radial diffusivity,RD);应用SSPS 17.0对T2DM、HCs组FA值、MD值及RD值数据进行统计学处理,并对T2DM组FA值、RD值与发病时间行Spearman相关性分析。结果T2DM组、HCs组额叶/颞叶FA值(P=0.041/0.012)、RD值存在统计学差异(P=0.010/0.015);T2DM组额叶/颞叶FA、RD值与发病时间(P=0.491,r=0.19;P=0.958,r=-0.14/P=0.312,r=-0.27;P=0.307,r=-0.28)无相关性。结论DTI技术可客观评价T2DM患者脑白质微观结构改变,为临床进行早期干预提供参考信息。Objective To detect the changes of white matter fibers in type 2 diabetes mellitus patients without cognitive impairment by diffusion tensor imaging.Methods Twenty patients with type 2 diabetes mellitus(T2DM)and 20 healthy controls(HCs)matched by age and sex were enrolled.Mini Mental State Examination(MMSE)was scored by specialists in both the patient group and the control group.Before MRI examination,fasting blood glucose and glycosylated hemoglobin were examined in laboratory.The sequence of MR examination included DTI,conventional MR(T1 Flair,T2WI,T2 Flair,DWI,excluding cerebrovascular diseases,tumors and other organic diseases).After MRI examination,the DTI sequence was post-processed by GE-ADW4.6 workstation to obtain ROI(region of interest,ROI)FA(fractional anisotropy,FA),MD(mean diffusivity,MD)and RD(radial diffusivity,RD)values of the region of interest;and the FA,MD and RD values of the T2DM and HCs groups were measured by SSPS 17.0.The correlation between FA,RD and onset time in patients with T2DM was analyzed by Spearman.Results There were statistical differences in FA and RD values of frontal and temporal lobes in T2DM and HCs groups(P=0.041/0.012)and there was no correlation between FA and RD values of frontal and temporal lobes and onset time(P=0.491,r=0.19;P=0.958,r=-0.14/P=0.312,r=-0.27;P=0.307,r=-0.28).Conclusion DTI can objectively evaluate the changes of white matter microstructure in patients with T2DM,and provide reference information for early clinical intervention.
