摘　　要：目的:探讨蛋白酶原(PG)、胃泌素、Hp检测联合ME-NBI诊断早期胃癌及癌前病变的价值。方法:选取2019年12月-2020年10月上消化道症状患者160例,随机分为白光内镜组及放大内镜组,各80例。按内镜及病理确诊分为非萎缩组、萎缩肠化组、异型增生组及早期胃癌组,检测患者的Hp(C14)感染情况、血清胃泌素及PG水平,与胃镜检查、病理诊断结果比较。结果:血清PG、胃泌素及Hp联合放大内镜联合检测的灵敏度、特异度与诊断符合率均高于任何单一筛查结果,差异有统计学意义(P<0.05)。结论:ME-NBI胃镜筛查早期胃癌的准确性及灵敏度较高,联合胃PG、胃泌素、Hp检测筛查胃癌,能够进一步提高检查灵敏度及诊断率。Objective:To investigate the value of PG,gastrin,Hp combined with ME-NBI in the diagnosis of early gastric cancer and precancerous lesions.Methods:From December 2019 to October 2020,160 patients with upper gastrointestinal symptoms were selected,they were randomly divided into the white light endoscopy group and the magnifying endoscopy group with 80 cases in each group.According to endoscopic and pathological diagnosis,they were divided into the non-atrophy group,the atrophic intestinal metaplasia group,the dysplasia group and the early gastric cancer group.The Hp(C14)infection,serum gastrin and pepsinogen(PGⅠ,PGⅡ,PGR)were detected,and compared with the results of gastroscopy and pathological diagnosis.Results:The sensitivity,specificity and diagnostic coincidence rate of combined detection of serum PG,gastrin and Hp combined with magnifying endoscopy were significantly higher than any single screening result,the difference was statistically significant(P<0.05).Conclusion:ME-NBI gastroscope has high accuracy and sensitivity in screening early gastric cancer.Combined with pepsinogen,gastrin,and Hp detection for gastric cancer screening,it can further improve the sensitivity and diagnosis rate.
