五情相胜疗法在老年患者胆囊术后情志护理中的运用

摘　　要：目的观察五情相胜疗法在老年患者胆囊术后情志护理中的干预效果。方法按随机数字表法将荆门市中医医院2018年11月—2020年12月住院行腹腔镜胆囊切除术的100例老年患者,分为干预组和对照组,每组50例。2组均行围手术期“常规护理+”模式,对照组加一般情志护理,而干预组则加五情相胜法的个体化情志护理。比较2组患者的术后首次下床时间、首次排气时间、住院天数及术后疼痛程度。术后疼痛程度采用视觉疼痛模拟量表(Visual analogue scale,VAS)观察2组术后第0、1、5、7天的VAS疼痛评分。结果干预组患者首次下床时间、首次排气时间、住院天数均显著少于对照组(P<0.05);干预组患者术后第1、5、7天(除术后当天外)VAS疼痛评分值明显低于对照组(P<0.05)。结论五情相胜法在老年胆囊术后患者情志护理中的应用效果良好,能显著减轻老年患者术后的疼痛,改善患者的消化道症状,缩短住院时间。Objective To observe the effect of psychotherapy in elderly cholecystectomic patients.Methods According to the random number table method,100 elderly patients who underwent laparoscopic cholecystectomy in Jingmen Hospital of Traditional Chinese Medicine from November 2018 to December 2020 were divided into the intervention group and the control group,with 50 cases in each group.The two groups were treated with routine nursing and mode during perioperative period.The control group was treated with general emotional nursing,while the intervention group was treated with individual psychotherapy.The first postoperative time of getting out of bed,first exhaust time,length of stay and postoperative pain were compared between the two groups.Postoperative pain degree Visual analogue Scale(VAS)was used to observe the VAS pain score at 0,1,5 and 7 days after surgery.Results The first time of getting out of bed,the first exhaust time and the length of hospitalization in the intervention group were significantly lower than those in the control group(P<0.05).VAS pain score in intervention group was significantly lower than that in control group on postoperative day 1,5 and 7(except postoperative day)(P<0.05).Conclusion The application effect of psychotherapy in the emotional nursing of elderly patients after cholecystectomy is good,which can significantly reduce the pain of elderly patients after surgery,improve the symptoms of the digestive tract,and shorten the hospital stay.
