Ki-67在三阴性与HER-2过表达型乳腺浸润性导管癌中的表达及临床意义

摘　　要：目的分析Ki-67在三阴性和人类表皮生长因子受体2(HER-2)过表达型乳腺浸润性导管癌(IDC)中的表达及其与患者临床病理特征、术后生存情况的关系。方法选取2011年1月至2019年10月在嘉兴市妇幼保健院乳腺科行手术治疗,术后病理证实为乳腺IDC患者269例,其中三阴性型160例,HER-2过表达型109例。所有患者均随访至2020年1月或患者死亡。比较三阴性与HER-2过表达型乳腺IDC中Ki-67表达情况(≥25%为高表达,<25%为低表达)、患者临床病理特征,分析Ki-67表达情况与临床病理特征、术后生存情况的关系。结果三阴性乳腺IDC患者与HER-2过表达型乳腺IDC患者Ki-67表达情况及年龄、月经状态、病灶直径、组织学分级、腋窝淋巴结转移情况等临床病理特征比较差异均无统计学意义(均P>0.05)。三阴性乳腺IDC患者Ki-67表达情况与月经状态无关(P>0.05),与年龄、病灶直径、组织学分级、腋窝淋巴结转移情况均有关(均P<0.05)。HER-2过表达型乳腺IDC患者Ki-67表达情况与年龄、月经状态无关(均P>0.05),但与病灶直径、组织学分级、腋窝淋巴结转移情况均有关(均P<0.05)。Ki-67高表达与低表达三阴性、HER-2过表达型乳腺IDC患者术后生存情况比较差异均无统计学意义(均P>0.05)。结论三阴性与HER-2过表达型乳腺IDC中Ki-67高表达与患者临床病理特征有一定关系,Ki-67表达情况可作为评估这两类乳腺癌病情的指标。Objective To investigate the Ki-67 expression in triple-negative breast invasive ductal carcinoma(IDC)and in HER-2 overexpressed breast IDC,and its correlations with clinicopathological characteristics and post-operative survival of patients.Methods A total of 269 patients with breast IDC treated at the department of breast diseases,Jiaxing Women and Children Hospital between January 2011 and October 2019 were enrolled in the study,including 160 cases of triple-negative IDC and 109 cases of HER-2 overexpressed IDC.All patients were followed up to January 2020 or death.The expression of Ki-67 was compared between the triple-negative IDC patients and the HER-2 overexpressed patients.The correlations of Ki-67 expression with clinicopathological characteristics and post-operative survival of patients was analyzed.Results There were no significant differences in Ki-67 expression,age,menstrual status,lesion diameter,histological grading,axillary lymphnode metastasis and other clinicopathological characteristics between the triple-negative IDC patients and the HER-2 overexpressed patients(P>0.05).For the triple-negative IDC patients,Ki-67 expression was not correlated with menstrual status(P>0.05)but correlated with age,lesion diameter,histological grading and axillary lymphnode metastasis(P<0.05).For the HER-2 overexpressed IDC patients,Ki-67 expression was not correlated with age and menstrual status(P>0.05)but correlated with lesion diameter,histological grading and axillary lymphnode metastasis(P<0.05).For both the triple-negative IDC and the HER-2 overexpressed patients,the expression of Ki-67 was not significantly correlated with post-operative survival(P>0.05).Conclusion The high expression of Ki-67 was correlated,to a certain extent,with clinicopathological characteristics in both the triple-negative IDC and the HER-2 overexpressed patients.The expression of Ki-67 may serve as indicators for evaluation the status of these two types of breast cancer.
