摘 要:目的本文采用静息态脑功能MRI影像技术研究轻微型肝性脑病患者局部脑功能后扣带回与双侧海马间功能连接变化情况,为确诊轻微型肝性脑病发病和病理机制提供科学可靠的影像学依据。方法选取2018年1月至2020年2月我院收治的70例轻微型肝性脑病患者作为病例组,另选取同期来做健康检查的体检者70例作为健康组,采用静息态脑功能MRI影像技术观察病例组患者在静息态下后扣带回与双侧海马间功能连接情况,评估两组患者的数字符号实验(DST)评分和数字连接实验-A(NCT-A)评分。结果病例组患者的NCT-A评分明显高于健康组,但DST评分明显低于健康组,组间差异有统计学意义(P<0.05)。与健康组相比,病例组右侧海马与后扣带回无FC明显增强的脑区,却存在FC减弱的区域(P<0.05)。右侧海马与后扣带回之间的FC分数与DST评分呈正相关,右侧海马与后扣带回之间的FC分数与NCT-A评分呈负相关,差异有统计学意义(P<0.05)。结论轻微型肝性脑病患者经过静息态功能MRI检查发现脑部有明显的脑损伤,后扣带回和双侧海马功能连接越强说明患者认知功能损伤越显著。Objective This paper uses static brain function MRI imaging technique to study local brain function and bilateral hippocampus functional connection in patients with mild hepatic encephalopathy,and provides a scientific and reliable imaging basis for confirming mild hepatic encephalopathy.Methods A total of 70 patients with mild hepatic encephalopathy admitted to our hospital from January 2018 to February 2020 were selected as the case group,and another 70 cases that came from health examinations in the same period were selected as the healthy group.The resting-state functional brain MRI imaging technique was used to observe the resting-state FC between posterior cingulate cortex and bilateral hippocampus in the patients of the case group.The digit symbol test(DST)score and the number connection test-A(NCT-A)score were assessed in both groups.Results Patients were significantly higher than in the health grou p,but significantly lower in the health group and were statistically significant(P<0.05).Compared with the health group,the right hippocampus in the case group returned to the brain area without significantly enhanced FC,but compared to areas with weakened FC(P<0.05).The FC score between the right hippocampus and postbuckback was positively correlated with the DST score,and the FC score between the right hippocampus and postbuckback was negatively correlated with the NCT-A score,which was statistically significant(P<0.05).Conclusion Patients with mild hepatic encephalopathy are found to have significant brain damage by resting-state functional MRI.The stronger the functional connectivity of the posterior cingulate cortex and bilateral hippocampus,the more significant the cognitive impairment of t he patients.
