榄香稀乳联合DNF化疗方案治疗晚期食管癌疗效观察

摘　　要：目的探讨榄香烯乳+多西他赛、奈达铂、氟尿嘧啶(DNF方案)化疗治疗晚期食管癌的临床效果。方法选取2018年2月至2020年5月间盐城市亭湖区人民医院收治的80例晚期食管癌患者,采用随机原则分为观察组和对照组,每组40例。对照组患者采用DNF化疗方案治疗,观察组患者在此基础上联用榄香烯乳治疗,比较两组患者的临床疗效、不良反应及生活质量。结果治疗前,两组患者癌胚抗原(CEA)和鳞状上皮细胞癌相关抗原(SCC)比较,差异无统计学意义(P>0.05)。治疗后,两组患者CEA和SCC均降低,且观察组均低于对照组,差异均有统计学意义(均P<0.05)。观察组患者治疗总有效率为62.5%,高于对照组的40.0%,差异有统计学意义(P<0.05)。两组患者不良反应发生情况比较,差异无统计学意义(P>0.05)。观察组患者身体功能、情绪功能、认知功能、社会功能、总体健康状况/生活质量和物理症状评分及总分均低于对照组,差异均有统计学意义(均P<0.05)。结论晚期食管癌患者采用榄香烯乳联合DNF化疗方案治疗,抗癌效果较好,不良反应尚可,值得临床深入研究后应用。Objective To study the clinical effect of elemene emulsion plus docetaxel,nedaplatin and fluorouracil(DNF chemotherapy)for advanced esophageal cancer.Methods Eighty patients with advanced esophageal cancer were selected at Tinghu District People's Hospital from February 2018 to May 2020 and randomly divided into an observation group and a control group with 40 patients in each group.The observation group was given elemene emulsion plus DNF chemotherapy and the control group was only given DNF chemotherapy.The clinical efficacy,adverse reactions and quality of life were compared between the two groups.Results There was no significant difference in carcinoembryonic antigen(CEA)and squamous cell carcinoma antigen(SCC-Ag)between the two groups before treatment(P>0.05).In contrast,the CEA and SCC levels decreased in the two groups after the treatment with the observation group lower than the control group(all P<0.05).The overall efficacy rate was 62.5%in the observation group which was higher than 40.0%of the control group(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).The scores of physical,emotional,cognitive and social function,overall health status/quality of life and physical symptoms were lower in the observation group than in the control group(all P<0.05).Conclusion Elemene emulsion plus DNF chemotherapy has significant anti-cancer efficacy and the adverse reactions are acceptable in patients with advanced esophageal cancer,which is worthy of further clinical study and clinical application.
