摘 要:目的:探讨MRI动态增强在非肿块样强化乳腺癌诊断中的价值。方法:回顾性分析2018年1月-2020年12月本院收治的32例MRI表现为非肿块样强化乳腺癌患者的临床资料和MRI图像,包括平扫T1WI、T2WI压脂及动态增强扫描,分析MRI图像上病灶的强化分布、时间-信号强度曲线等。结果:32例患者经手术病理证实为乳腺癌,其中导管内癌9例(28.1%),浸润性导管癌18例(56.3%),浸润性导管癌及小叶癌5例(15.6%);32例病例中3例呈线样分布(9.4%)、17例呈段样分布(53.1%)、8例呈区域样分布(25.0%)、4例呈多区域样分布(12.5%);32例病例中21例TIC呈上升-平台型(65.6%),11例呈速升速降型(34.4%);32例患者中13例出现乳导管扩张(40.6%)。结论:非肿块样强化乳腺癌MRI动态增强主要表现为段样分布,部分伴有乳导管扩张,具有一定特征性,TIC多呈上升平台型,MRI动态增强通过血供特征显示病灶,较超声敏感性高,有助于明确诊断。Objective To investigate the value of dynamic MRI enhancement in the diagnosis of non-mass enhanced breast cancer.Methods The clinical data and MRI images of 32 patients with breast cancers showing non-mass enhancement were retrospectively analyzed including plain T1WI,fat suppressed T2WI and dynamic enhanced T1WI.And the enhancement distribution and timesignal intensity curve of the lesions were analyzed.Results 32 patients were pathologically confirmed as breast cancers including 9 intraductal carcinomas(28.1%),18 infiltrative ductal carcinomas(56.3%)and 5 infiltrative ductal carcinoma and lobular carcinomas(15.6%).Among them,3 showed linear distribution(9.4%),17 showed segmental distribution(53.1%),8 showed regional distribution(25.0%)and 4 showed multi-regional distribution(12.5%).Among the 32 cases,21(65.6%)showed an ascending plateau type of TIC,and 11(34.4%)showed a rapidly ascending and decreasing type of TIC.13 of 32 patients duct dilatation(40.6%).Conclusion In dynamic MRI enhancement,the non-mass enhanced breast cancer mainly shows segmental distribution,and some of them are accompanied with ductal dilatation.The TIC is mostly the ascending plateau type.The dynamic MRI enhancement reflects the characteristics of blood supply,which is more sensitive than ultrasound,and helpful for particular diagnosis.
