摘 要:目的:探讨2017版TI-RADS分类系统5类甲状腺结节与细针穿刺活检病理结果比较。方法:回顾性分析自2019年1月-2020年4月期间于商洛市中心医院行超声检查提示TI-RADS5类,并行细针穿刺活检取得病理学结果的64个甲状腺结节术前超声特征,比较2017版TI-RADS分类5类甲状腺结节与术后病理结果。结果:64个TI-RADS5类结节中,恶性45个(70.3%),良性19个(29.7%)。45个恶性结节超声特征,低回声或极低回声实性结节(95.6%),纵横比>1(62.2%),边界不规则或向包膜外侵犯(77.8%),微小钙化(55.5%),颈部淋巴结转移(20.0%)。19个良性结节超声特征,实性低回声结节(100%),纵横比>1(52.6%),边界不规则或向外包膜外侵犯(36.8%),微小钙化(42.1%),颈部淋巴结转移(0)。结论:根据2017版TI-RADS分类系统,医师能够清晰认识5类甲状腺结节超声特征,提高超声诊断准确性,同时观察颈部淋巴结特征,为临床制定进一步诊疗方案提供必要方向。Objective To Compare of pathological results between 5 types of thyroid nodules in the 2017 TI-RADS classification system and fine needle aspiration biopsy.Methods A retrospective analysis of 64 cases with pathological results obtained by ultrasonography in Shangluo Central Hospital from January 2019 to April 2020 indicating TI-RADS category 5.The preoperative ultrasonographic features of 64 thyroid nodules with pathological results obtained by fine needle aspiration biopsy.Then,compare the 2017 TI-RADS classification of 5 types of thyroid nodules with postoperative pathological.Results Among the 64 TI-RADS category 5 nodules,45 were malignant(70.3%)and 19 were benign(29.7%).Among them,45 malignant nodules are hypoechoic or very hypoechoic solid nodules(95.6%),aspect ratio>1(62.2%),irregular borders or protruding outwards(77.8%)in terms of ultrasound imaging performance,microcalcification(55.5%),cervical lymph node metastasis(20.0%).19 benign nodules are solid hypoechoic nodules(100%),aspect ratio>1(52.6%),irregular borders or protruding outwards(36.8%),and microcalcifications(42.1%),cervical lymph node metastasis(0).Conclusion According to the 2017 version of the TI-RADS classification system,doctors can clearly understand the ultrasonic sonographic characteristics of the 5 types of thyroid nodules,so as to improve the accuracy of ultrasound diagnosis and observe the neck lymph node characteristics at the same time,and provide the necessary direction for clinical development of further diagnosis and treatment plans.
