胃超声造影及核磁共振在进展期胃癌术前TNM分期诊断的一致性分析

摘　　要：目的:研究胃充盈超声造影检查(GFUS)及核磁共振(MRI)在进展期胃癌术前TNM分期诊断的一致性。方法:回顾性分析2018年12月至2020年12月在河北中石油中心医院接受治疗的96例进展期胃癌患者临床资料,患者在术前一周行MRI及GFUS检查,并进行TNM分期评估,手术治疗后以术后病理检查结果为金标准,采用Kappa检验对两种检查方式术前TNM分期诊断一致性进行评估。结果:GFUS及MRI在进展期胃癌术前TNM分期诊断中均与术后病理检测结果有较高的一致性(P<0.05)。GFUS诊断进展期胃癌患者术前T1、T2、T3、T4分期的Kappa值分别为0.775、0.772、0.822、0.921,高于MRI诊断的0.699、0.702、0.698、0.700。GFUS诊断进展期胃癌患者术前N0、N1、N2、N3分期的Kappa值分别为0.862、0.789、0.783、0.817,高于MRI诊断的0.769、0.714、0.684、0.744。GFUS诊断进展期胃癌患者术前M0、M1分期的Kappa值分别为0.721、0.721,高于MRI诊断的0.612、0.612。结论:GFUS、MRI诊断进展期胃癌患者术前TNM分期均与术后病理检查结果有较高一致性,但GFUS一致性更高。Objective:To study the consistency of contrast-enhanced ultrasonography(GFUS)and magnetic resonance imaging(MRI)in preoperative TNM staging of advanced gastric cancer.Methods:The clinical data of 96 patients with advanced gastric cancer treated in our hospital from December 2018 to December 2020 were retrospectively analyzed.MRI and GFUS examination were performed one week before operation,and TNM staging was evaluated.After operation,the results of postoperative pathological examination were taken as the gold standard,Kappa test was used to evaluate the consistency of preoperative TNM staging.Results:GFUS and MRI in the preoperative TNM staging diagnosis of advanced gastric cancer were highly consistent with the postoperative pathological results(P<0.05).The Kappa values of preoperative T1,T2,T3 and T4 stages in patients with advanced gastric cancer diagnosed by GFUS were 0.775,0.772,0.822 and 0.921,respectively,which were higher than those diagnosed by MRI(0.699,0.702,0.698 and 0.700).The Kappa values of preoperative N0,N1,N2 and N3 stages in patients with advanced gastric cancer diagnosed by GFUS were 0.862,0.789,0.783 and 0.817,respectively,which were higher than those diagnosed by MRI(0.769,0.714,0.684 and 0.744).The Kappa values of preoperative M0 and M1 staging in patients with advanced gastric cancer diagnosed by GFUS were 0.721 and 0.721 respectively,which were higher than those diagnosed by MRI(0.612 and 0.612 respectively).Conclusion:The preoperative TNM staging of patients with advanced gastric cancer diagnosed by GFUS and MRI has higher consistency with the postoperative pathological examination results,but the consistency of GFUS is higher.
