摘 要:目的探讨实施助产士住院总工作模式培养低年资助产士的实践与效果。方法广东省妇幼保健院产科在2017年3月—2019年11月先后对33名N1级助产士开展助产士住院总工作模式培训,每3个月为1个周期,每期培训3名助产士,共开展11期。分析担任助产士住院总期间该33名助产士参与科室工作的情况,并对比培训前后该33名助产士核心能力考核情况、一般自我效能感得分。结果担任助产士住院总期间,该33名助产士积极投身参与科室工作,超额完成科室工作要求总量。实施助产士住院总培训模式后,助产士核心能力考核总得分[(80.98±1.55)分]高于实施前[(70.30±1.96)分],差异有统计学意义(P<0.001)。利用迷你临床演练评量(Mini-CEX)评核助产士单胎顺产接生操作,优秀率由45.45%(15/33)提升到90.91%(30/33),实施后助产士单胎顺产接生考核得分为优秀的占比高于实施前,差异有统计学意义(P<0.001)。实施后的一般自我效能感得分[(3.07±0.11)分]高于实施前[(2.05±0.15)分],差异有统计学意义(t=34.690,P<0.001)。结论实施住院总工作模式有助于改善低年资助产士的助产质量,能有效提高低年资助产士专科核心能力,并显著提升其自我效能感。Objective To explore the practice and effect of implementing the chief resident work model of midwives to cultivate junior midwives.Methods From March 2017 to November 2019,the Obstetrics Department of Guangdong Maternal and Child Health Hospital successively carried out midwife chief resident training for 33 N1 midwives,with each cycle of 3 months,training 3 midwives in each period.A total of 11 sessions were carried out.Their participations in the department during the chief resident period as a midwife were analyzed.The core competence assessment and general self-efficacy scores of the 33 midwives before and after the training were compared.Results During the chief resident period as a midwife,the 33 midwives actively participated in the work of the department and exceeded the total requirements of the department.After the implementation of the midwife chief resident training model,the total score of midwives′core competence assessment([80.98±1.55]points)was higher than that before the implementation([70.30±1.96]points),and the difference was statistically significant(P<0.001).Using the mini-clinical evaluation exercise(Mini-CEX)to evaluate the delivery of single birth by midwives,the excellent rate increased from 45.45%(15/33)to 90.91%(30/33),the proportion of midwives who scored excellence in the assessment of singleton deliveries after the implementation was higher than that before the implementation,and the difference was statistically significant(P<0.001).The general self-efficacy score after the implementation([3.07±0.11]points)was higher than that before the implementation([2.05±0.15]points),and the difference was statistically significant(t=34.690,P<0.001).Conclusion The implementation of chief resident work model is conductive to improve the midwifery quality for junior midwives,can effectively improve their core competency,and significantly enhance their self-efficacy.
