摘　　要：目的探讨进展性缺血性脑卒中(progressive ischemicstroke,PIS)患者血糖、C-反应蛋白变化特点及与预后的相关性。方法选取2018年1月至2019年1月收治的急性缺血性脑卒中患者180例,根据患者发病后6 h~1周的NIHSS评分分为PIS组(NIHSS评分≥2分,100例)和非PIS组(NIHSS评分<2分,80例)。比较2组血糖、C-反应蛋白变化特点。对PIS组随访90 d,并进行改良Rankin(mRS)评分,其中≤2分者44例为转归良好组,>2分者56例为转归不良组,收集2组临床资料,进行Logistic回归分析。结果非PIS组空腹血糖(FPG)、C-反应蛋白水平明显低于PIS组,差异有统计学意义(P<0.05);Logistic回归分析显示,FPG、年龄、C-反应蛋白、高脂血症是PIS的独立危险因素(P<0.05)。结论PIS的血糖水平以高血糖为主,且血糖、C-反应蛋白水平与PIS预后有明显相关性,早期监测血糖、C-反应蛋白对PIS治疗具有指导意义。Objective To investigate the changes of blood glucose level and C-reactive protein(CRP)in patients with progressive ischemic stroke(PIS)and their correlation with prognosis.Methods A total of 180 patients with acute ischemic stroke who were treated in our hospital from January 2018 to January 2019 were enrolled in the study,who were divided into PIS group(NIHSS score≥2,n=100)and non PIS group(NIHSS score<2,n=80)according to the NIHSS score 6 hours to 1 week after onset.The changes of blood glucose and CRP were observed and compared between the two groups.The patients in PIS group were followed up for 90 days,and the modified Rankin(MRS)score was performed,of whom,44 patients≤2 points were enrolled as good outcome group,and 56 patients>2 ponits were enrolled as poor outcome group.The clinical data of both groups were collected and analyzed by Logistic regression.Results The levels of FPG and CRP in non-PIS group were significantly lower than those in PIS group(P<0.05).Logistic regression analysis showed that FPG,CRP,hyperlipidemia and age were independent risk factors for PIS(P<0.05).Conclusion The blood glucose level of patients with PIS is mainly hyperglycemia,moreover,the the levels of blood glucose and CRP are significantly correlated with the prognosis of PIS,therfor the early monitoring of blood glucose and CRP levels has guiding significance for the treatment of PIS.
