微波热疗机热化疗对Ⅳ期消化道肿瘤患者疗效及血清Survivin蛋白和DLL4表达的影响

摘　　要：目的探讨微波热疗机热化疗对Ⅳ期消化道肿瘤患者临床疗效及血清凋亡抑制基因蛋白(Survivin)及Delta样配体4(DLL4)表达的影响。方法选取2015年4月至2016年12月收治的消化道肿瘤患者200例,按照随机数字表法分为对照组和观察组,每组100例。对照组采用奥沙利铂全身静脉化疗,观察组在对照组基础上联合深部热疗增敏治疗。比较2组患者疗效,免疫功能(CD^(+)_(4)、CD^(+)_(8)、CD^(+)_(4)/CD^(+)_(8)、NK),治疗前、治疗1个月、治疗6个月血清Survivin蛋白及DLL4表达,生存曲线及不良反应。结果对照组疾病控制率为45.0%,观察组疾病控制率为68.0%,观察组显著高于对照组(P<0.05);观察组患者CD^(+)_(4)、CD^(+)_(8)、CD^(+)_(4)/CD^(+)_(8)、NK显著高于对照组(P<0.05);观察组患者Survivin蛋白明显低于对照组(P<0.05);2组患者治疗前后DLL4比较差异有统计学意义(P<0.05),治疗前2组患者DLL4比较差异无统计学意义(P>0.05),治疗后2组患者DLL4显著降低(P<0.05),观察组患者DLL4明显低于对照组(P<0.05);观察组生存时间显著长于对照组(P<0.05)。结论微波热化疗能有效提高化疗患者临床疗效、免疫功能并下调血清Survivin蛋白及DLL4的表达,值得临床推广。Objective To investigate the effects of thermochemotherapy with microwave hyperthermia machine on the clinical efficacy and the expressions of serum survivin protein and Dll4 in patients with stageⅣgastrointestinal tumor.Methods Two hundred patients with gastrointestinal tumor who were treated in our hospital from April 2015 to December 2016 enrolled in the study,who were randomly divided into control group and observation group,with 100 cases in each group.The patients in control group were treated by chemotherapy with oxaliplatin,however,the patients in observation group,on the basis of control group,were treated by deep thermotherapy.The general information(age,gender,tumor type,functional status score),curative effects,immune function(CD^(+)_(4),CD^(+)_(8),CD^(+)_(4)/CD^(+)_(8),NK),serum levels of Survivin protein and DLL4,and survival curve and adverse reactions were observed and compared between the two groups.Results The disease control rate in observation group was 68%,which was significantly higher than that(45%)in control group(P<0.05).After treatment,the levels of CD^(+)_(4),CD^(+)_(8),CD^(+)_(4)/CD^(+)_(8),NK in observation group were significantly higher than those in the control group(P<0.05),however,the levels of Survivin protein in observation group were significantly lower than those in control group(P<0.05).After treatment the levels of Dll4 in both groups were significantly decreased(P<0.05),moreover,which in observation group were significantly lower than those in control group(P<0.05).In addition the survival curve in observation group was significantly longer than that in control group(P<0.05),and no severe adverse reactions were found in both groups.Conclusion Microwave hyperthermic chemotherapy can effectively improve the clinical efficacy,immune function and down regulate the expressions of serum survivin protein and Dll4 in patients with stageⅣgastrointestinal tumor,therefor,which is worthy of clinical promotion.
