摘　　要：目的探讨食管癌患者电视胸腔镜手术后胸腔出血发生的影响因素。方法选取2016年1月至2021年1月于新郑市人民医院接受电视胸腔镜手术治疗的240例食管癌患者为研究对象,观察患者手术治疗后胸腔出血发生情况,并收集患者临床资料,分析可能影响食管癌患者电视胸腔镜手术后发生胸腔出血的相关因素。结果240例食管癌患者电视胸腔镜手术后发生胸腔出血34例(14.17%),未发生胸腔出血206例(85.83%);发生组吸烟史、肿瘤直径、病灶浸润深度、血小板计数与未发生组比较,差异有统计学意义(P<0.05);经多元logistic回归模型结果显示,吸烟史、肿瘤直径≥3 cm、病灶浸润深度为M3或SM1、血小板计数低是食管癌患者电视胸腔镜手术后发生胸腔出血的影响因素(OR>1,P<0.05)。结论食管癌患者电视胸腔镜手术后胸腔出血发生率较高,吸烟史、肿瘤直径≥3 cm、病灶浸润深度为M3或SM1、血小板计数低是食管癌患者电视胸腔镜手术后发生胸腔出血的影响因素。Objective To investigate the influencing factors of thoracic hemorrhage in patients with esophageal cancer after video assisted thoracic operation.Methods A total of 240 patients with esophageal cancer who underwent video assisted thoracic operation in Xinzheng People’s Hospital from January 2016 to January 2021 were selected as the research subjects.The occurrence of thoracic hemorrhage of patients after surgical treatment was observed.Clinical datas of patients were collected,and the related factors that might affect the occurrence of thoracic hemorrhage in patients with esophageal cancer after video assisted thoracic operation were analyzed.Results Among 240 patients with esophageal cancer after video assisted thoracic operation,34 cases(14.17%)had thoracic hemorrhage,and 206 cases(85.83%)did not have thoracic hemorrhage.Compared smoking history,tumor diameter,depth of invasion and platelet count between the occurrence group and the non occurrence group,the differences were statistically significant(P<0.05).By multiple logistic regression model,the results showed that smoking history,tumor diameter≥3 cm,depth of invasion M3 or SM1,low platelet count were the influencing factors of thoracic hemorrhage in patients with esophageal cancer after video assisted thoracic operation(OR>1,P<0.05).Conclusion The incidence of thoracic hemorrhage in patients with esophageal cancer after video assisted thoracic operation is high,smoking history,tumor diameter≥3 cm,depth of invasion M3 or SM1,low platelet count are the influencing factors of thoracic hemorrhage in patients with esophageal cancer after video assisted thoracic operation.
