摘 要:基于肝癌微环境防治肝癌是新的策略,其中肝再生微环境与肝癌的相关机制研究提高了人们对肝癌发生发展、复发转移机制的认识和防治水平。正常的肝再生微环境可延缓、阻止、甚或逆转肝癌的发生发展或复发转移,而异常的或恶化的肝再生微环境则会启动、促进、甚或加速肝癌的发生发展或复发转移。肝癌的肿瘤异质性主要表现为治疗后出现的第二原发肿瘤、不同克隆性的同步多灶性肿瘤以及表型和遗传水平的瘤内异质性,为肝癌分子靶向药物的开发增加了极大的复杂度。根据肝癌肝再生微环境与肝癌肿瘤异质性的研究进展,提出肝癌肝再生微环境具有影响肝干细胞恶性转化肝癌及其异质性的作用及其机制的科学假说,拟解决的关键科学问题之一是肝癌肝再生微环境影响肝癌肿瘤异质性(肝癌肝再生微环境影响肝干细胞恶性转化肝癌及其异质性)的相关机制。The prevention and treatment of hepatocellular carcinoma(HCC)based on the tumor microenvironment is a new strategy,among which the research on the mechanism of liver regeneration microenvironment and hepatocellular carcinoma has improved the understanding of the occurrence,development,recurrence and metastasis of hepatocellular carcinoma and the level of prevention and treatment.Normal liver regenerative microenvironment delays,prevents,or even reverses the occurrence,development,recurrence and metastasis of HCC,while abnormal or deteriorated liver regenerative microenvironment initiates,promotes,or even accelerates the occurrence,development,recurrence and metastasis of HCC.The tumor heterogeneity of HCC is mainly manifested by secondary primary tumors after treatment,synchronous multifocal tumors with different clones,and intratumoral heterogeneity at phenotypic and genetic levels,which adds great complexity to the development of molecular targeted drugs for HCC.Based on the research progress of liver regenerative microenvironment of HCC and the heterogeneity of HCC tumors,we proposed the scientific hypothesis that the liver regenerative microenvironment of HCC can influence the role of liver stem cells in malignant transformation of HCC and its heterogeneity and its mechanism.One of the key scientific questions is the mechanism by which HCC microenvironment influences HCC tumor heterogeneity(HCC microenvironment influences HCC malignant transformation and HCC heterogeneity).
