摘 要:目的了解近3年绵阳市部分体检儿童整体维生素D营养状态以及新型冠状病毒肺炎(以下简称"新冠肺炎")疫情期间居家防护下维生素D营养状态的改变,为疫情后本地区儿童维生素D的监测及合理补充提供理论依据。方法回顾性分析2018年1月—4月、2019年1月—4月、2020年1月—4月3个时段内于绵阳市中心医院儿童保健科进行体检儿童的临床资料。采用高效液相色谱串联质谱法检测儿童血清维生素D组分,观察维生素D2、D3及25-羟基维生素D[25-hydroxyvitamin D,25(OH)D]在不同性别、年龄段及不同年度的差异。结果共纳入儿童12348名。维生素D2平均为(4.89±6.02)ng/mL,维生素D3平均为(22.91±9.29)ng/mL,25(OH)D平均为(27.81±10.53)ng/mL,25(OH)D充足者9434人。2018年维生素D2、维生素D3、25(OH)D及25(OH)D营养状态,2019年维生素D2及25(OH)D,2020年维生素D2在不同性别间比较,差异均无统计学意义(P>0.05)。2019年维生素D3及25(OH)D营养状态,2020年维生素D3、25(OH)D及25(OH)D营养状态在不同性别间比较,差异均有统计学意义(P<0.05)。2018年—2020年,婴儿组维生素D2最高(P<0.05),幼儿组维生素D3、25(OH)D、25(OH)D营养状态均最高(P<0.05)。2018年—2020年,维生素D2(χ^(2)=143.106,P<0.001)整体呈下降趋势,维生素D3(F=400.178,P<0.001)、25(OH)D(F=447.384,P<0.001)整体均呈上升趋势;25(OH)D营养状态在2019年最高(χ^(2)=103.566,P<0.001)。结论绵阳地区儿童整体维生素D营养状况尚可。在居家防护下,儿童25(OH)D平均水平变化不大,但维生素D营养状态下降明显。新冠肺炎疫情暴发后应关注学龄期女童维生素D的监测与补充。Objective To understand the nutritional status of vitamin D in some children aged 0-14 in Mianyang during the past 3 years and the changes of vitamin D nutritional status under home protection during the coronavirus disease 2019(COVID-19)epidemic,so as to provide a theoretical basis for the monitoring and reasonable supplementation of vitamin D in children in this area after the epidemic.Methods The clinical data of children aged 0-14 who underwent physical examination in the Children’s Health Department of Mianyang Central Hospital from January to April 2018,from January to April 2019 and from January to April 2020 were analyzed retrospectively.High performance liquid chromatography tandem mass spectrometry was used to detect vitamin D,including vitamin D2,vitamin D3 and 25-hydroxyvitamin D[25(OH)D]in children’s serum.The differences in vitamin D components and25(OH)D between different genders,different age groups,and different years were analyzed.Results A total of 12348 children were included.The average vitamin D2 was(4.89±6.02)ng/mL,the average vitamin D3 was(22.91±9.29)ng/mL,the average 25(OH)D was(27.81±10.53)ng/mL,and 9434 cases had sufficient 25(OH)D.The differences in vitamin D2,vitamin D3,25(OH)D and 25(OH)D nutritional status in 2018,vitamin D2 and 25(OH)D in 2019,and vitamin D2 in 2020 between different genders were not statistically significant(P>0.05).There were statistically significant differences in vitamin D3 and 25(OH)D nutritional status in 2019,vitamin D3,25(OH)D and 25(OH)D nutritional status in 2020 between different genders(P<0.05).From 2018 to 2020,vitamin D2 was the highest in infant group(P<0.05),while vitamin D3,25(OH)D and 25(OH)D nutritional status were the highest in children group(P<0.05);vitamin D2(χ^(2)=143.106,P<0.001)showed an overall downward trend,vitamin D3(F=400.178,P<0.001)and 25(OH)D(F=447.384,P<0.001)showed an overall upward trend;25(OH)D nutritional status(χ^(2)=103.566,P<0.001)was the highest in 2019.Conclusions The overall vitamin D nutritional status of chi
