摘  要:目的了解膳食炎症指数(dietary inflammatory index, DII)与妊娠期糖尿病(gestational diabetes mellitus, GDM)发生的关系,及DII与中性粒/淋巴细胞比值(neutrophil to lymphocyte ratio, NLR)、血糖、血脂的相关性。方法选择2019年10月—2020年8月门诊确诊的82例GDM患者作为观察组,82例同期健康孕妇作为对照组。比较两组DII评分,采用多因素logistic回归方法分析孕中期DII与GDM的相关性。采用Pearson相关方法分析DII与NLR、空腹血糖、总胆固醇、甘油三酯的相关性。结果 GDM组DII评分显著高于对照组,差异有统计学意义(P<0.01)。根据DII评分中位数将DII分为抗炎倾向组(DII<-0.76)和促炎倾向组(DII≥-0.76), GDM发生率分别为13.41%、86.58%,差异有统计学意义(χ^(2)=4.643,P=0.027)。Pearson相关分析显示,DII水平与NLR、空腹血糖、总胆固醇、甘油三酯呈正相关,相关系数分别为:0.772、0.879、0.203、0.219,差异有统计学意义(P<0.05)。GDM组每日促炎营养素摄入量高于对照组,而每日抗炎营养素摄入量低于对照组。结论 DII评分可作为GDM患者的一个监测指标,通过调整饮食结构降低DII,或可降低血清NLR、空腹血糖、总胆固醇、甘油三酯水平,从而降低GDM及其并发症的发病率。Objective To understand the relationship between dietary inflammatory index(DII)and the occurrence of gestational diabetes mellitus(GDM)as well as the correlation of DII with neutrophil to lymphocyte ratio(NLR),blood glucose and lipid profile.Methods Eighty-two patients with GDM diagnosed in clinic from October 2019 to August 2020 were selected as the observation group,while 82 healthy pregnant women in the same period served as the control group.The DII scores of the two groups were compared,and the correlation between DII during the second trimester of pregnancy and GDM was analyzed by multivariate logistic regression.Pearson correlation analysis was used to analyze the correlation of DII with NLR,fasting blood glucose,total cholesterol and triglyceride.Results The DII score was significantly higher in the GDM group than in the control group,with a statistically significant difference(P<0.01).According to the median DII score,DII was divided into the anti-inflammatory prone group(DII<-0.76)and the pro-inflammatory prone group(DII≥-0.76),and the incidence rates of GDM were 13.41%and 86.58%,respectively,showing statistically significant differences(χ^(2)=4.643,P=0.027).Pearson correlation analysis revealed that the DII level was positively correlated with NLR,fasting blood glucose,total cholesterol and triglyceride,with correlation coefficients of 0.772,0.879,0.203 and 0.219,respectively,showing statistically significant differences(P<0.05).The daily intake of pro-inflammatory nutrients was higher in the GDM group than in the control group,whereas the daily intake of anti-inflammatory nutrients was lower in the GDM group than in the control group.Conclusion The DII score can be used as a monitoring index for GDM patients.Reducing DII by adjusting diet structure may decrease the levels of serum NLR,fasting blood glucose,total cholesterol and triglyceride so as to decline the incidence of GDM and its complications.
