摘　　要：目的观察滋阴扶正解毒汤辅助新辅助放化疗治疗直肠癌的减毒增效效应。方法将78例直肠癌患者按照随机对照的原则分为2组,对照组39例予三维适形放疗联合卡培他滨片化疗治疗,治疗组39例在对照组治疗基础上加服滋阴扶正解毒汤治疗。6周后比较2组近期疗效,骨髓抑制、放射性肠炎发生情况,T淋巴细胞亚群及生活质量变化情况。结果治疗组临床控制率为64.10%(25/39),对照组为56.41%(22/39),治疗组临床控制率高于对照组。治疗组放射性肠炎及骨髓抑制发生等级均低于对照组(P<0.05)。治疗后对照组CD3^(+)较本组治疗前下降(P<0.05),治疗组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平均较本组治疗前升高(P<0.05),CD8^(+)较本组治疗前降低(P<0.05),且CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)高于对照组(P<0.05)。治疗后治疗组欧洲癌症研究与治疗组织生命质量测定量表(EORTC QLQ-C30)功能领域各项评分及总体健康状况评分均较本组治疗前明显升高(P<0.05),且高于对照组治疗后(P<0.05),对照组功能领域中躯体功能、认知功能、社会功能、情绪功能及总体健康状况评分均较本组治疗前升高(P<0.05);治疗后2组症状领域各项评分均较本组治疗前降低(P<0.05),且治疗组低于对照组(P<0.05)。结论滋阴扶正解毒汤辅助新辅助放化疗治疗直肠癌能提高近期疗效,减轻放射性肠炎及骨髓抑制的发生,改善机体免疫功能,提高患者的生活质量,具有减毒增效的效应。Objective To observe the effect of Ziyin Fuzheng Jiedu Decoction combined with neoadjuvant chemoradiotherapy for rectal cancer(RC).Methods Totally 78 RC patients were randomly assigned into control group(n = 39) and treatment group(n = 39),patients in former group were treated with neoadjuvant chemoradiotherapy,and patients in latter were additionally treated with Ziyin Fuzheng Jiedu Decoction on the basis of former group.Aiming to compare Short-term efficacy,the incidences ofradiation enteritis andbone marrow suppression,the change of t lymphocyte subset and quality-of-life between groups after 6-week treatment.Results The clinical control rate in treatment group was better than those in control group[64.10%(25/39) vs 56.41%(22/39),(P < 0.05),respectively].The incidences of radiation enteritis and bone marrow suppression were lower in treatment group than in control group(P < 0.05).Compared with before treatment,CD3+in control group was decreased after treatment(P < 0.05);CD3+,CD4+,CD4+/CD8+in treatment group was increased after treatment(P < 0.05),while CD8+was decreased(P < 0.05);and CD3+,CD4+,CD4+/CD8+in treatment group were higher than those in control group after treatment(P < 0.05).Compared with before treatment,scores,which met European Organization for Research and Treatment of Cancer QLQ-C30(EORTC QLQ-C30),and health evaluation score in treatment group were significantly increasedafter treatment(P < 0.05),which were higher than those in control group(P < 0.05);functional areas including physical function, cognitive function, social function,emotional function, as well as health evaluation score after treatment in control group were significantly increased(P <0.05);TCM symptom scores after treatment in groups were lower(P < 0.05),which in treatment group were significantly decreased(P < 0.05).Conclusion For RC patients,Ziyin Fuzheng Jiedu Decoction combined with neoadjuvant chemoradiotherapy can improve short-term effects,effectively reduce the occurrence of radiation enteritis and bone marrow suppressio
