摘  要:目的探讨电针结合康复训练通过SDF-1/CXCR4信号通路对癫痫大鼠的保护机制。方法 70只雄性SD大鼠随机分为对照组(n=10)、模型组(n=30)、治疗组(n=20)与中和组(n=10),采用氯化锂联合戊四氮致构建癫痫大鼠模型,观察癫痫大鼠认知功能、癫痫发作频次、持续时间;TUNEL方法检测癫痫大鼠海马区凋亡程度;ELISA方法检测大鼠血清SDF-1水平;Western blot、qPCR与免疫组织化学方法检测各组大鼠脑组织SDF-1和CXCR4表达的变化。结果癫痫大鼠予以电针结合康复训练治疗后,走平衡木评分、转动转棒行走评分、抓握网屏试验评分、癫痫发作次数和持续时间均明显降低(P<0.05);大鼠癫痫程度IV级、V级的发生率明显低于模型组(P<0.05);大鼠血清和脑组织SDF-1、CXCR4的表达水平明显升高(P<0.05);大鼠海马组织神经细胞凋亡明显减少(P<0.05)。进一步给予SDF-1中和抗体后,发现电针结合康复训练组大鼠的上述神经运动功能评分、癫痫发生次数和持续时间均显著延长(P<0.05)。结论电针结合康复训练可通过激活SDF-1/CXCR4信号通路发挥癫痫大鼠的保护作用。Objective To explore the protective mechanism of SDF-1/CXCR4 on epileptic rats. Methods Seventy male SD rats were randomly divided into control group(n=10), model group(n=30), treatment group(n=20) and neutralization group(n=10). The epileptic rat model was established by lithium chloride combined with pentylenetetrazol. The cognitive function, seizure frequency and duration of epileptic rats were observed. TUNEL immunohistochemistry was used to detect the apoptosis rate.The serum levels of SDF-1 were detected by ELISA, and the expression of SDF-1 and CXCR4 were detected by Western blot, qPCR and immunohistochemistry. Results After electroacupuncture combined with rehabilitation training, the scores of walking balance beam, rotating rod walking, grasping screen test, seizure times and duration of epileptic rats were significantly reduced(P<0.05);the incidence of epilepsy grade IV and V was significantly lower than that of model group. The expression levels of SDF-1 and CXCR4 in serum and brain tissue were significantly increased(P<0.05), and the apoptosis of neurons in hippocampus was significantly decreased(P<0.05). After further administration of SDF-1 neutralizing antibody, it was found that the above neuromotor function scores, the frequency and duration of epilepsy were significantly prolonged in the electroacupuncture combined with rehabilitation training group(P<0.05). Conclusion Electroacupuncture combined with rehabilitation training can play a protective role in epileptic rats by activating SDF-1/CXCR4 signaling pathway.
