摘 要:目的探讨乳腺癌患者血清可溶性CD146[又称为黑色素瘤细胞粘附分子(MCAM)](sCD146)水平与肿瘤组织MCAM基因甲基化状态及患者临床病理特征之间的关系。方法收集2019年5月至2020年5月在我院确诊并接受手术治疗的148例原发性乳腺癌患者的新鲜肿瘤组织和癌旁组织以及术前血液样本,另收集30例健康体检人群的血液样本作为正常对照组。采用甲基化定量PCR技术(qMSP)确定组织中MCAM基因甲基化程度,并采用ELISA法检测血清sCD146水平。用Pearson法分析乳腺癌患者两项指标之间的相关性。结果对于乳腺癌患者,血清sCD146水平高于正常对照组人群(P<0.05),且乳腺癌组织MCAM基因甲基化程度的中位数明显低于相应的癌旁组织(P<0.05)。血清sCD146水平与肿瘤组织中MCAM基因甲基化程度呈负相关性(r=-0.461,P<0.001)。此外,血清sCD146水平与肿瘤组织中MCAM基因甲基化程度与肿瘤T分期、分级、Ki-67表达及雌激素受体(ER)、孕激素受体(PR)表达有关(P<0.05)。而且三阴性乳腺癌患者血清sCD146水平和组织MCAM基因低甲基化率最高,显著高于Luminal A型和Luminal B-HER2-型(P<0.05)。结论乳腺癌患者血清sCD146水平较健康对照人群普遍升高,以三阴性乳腺癌患者升高最明显,这可能与肿瘤组织中MCAM基因发生去甲基化有关;而且血清sCD146水平也随着肿瘤T分期和组织学分级增加而逐渐升高。Objective To analyze the correlation of soluble CD146[also named as melanoma cell adhesion molecule(MCAM)](sCD146)level and MCAM gene methylation status and clinical pathological features in breast cancer patients.Methods 148 cases of breast cancer from 05/2019 to 05/2020 in our hospital were selected and the tumor and adjacent normal breast tissue were collected after surgery.The promoter methylation status of MCAM in these tissues was detected by quantitative methylation specific PCR(qMSP).The blood samples before surgery from breast cancer patients or 30 healthy volunteers(control group)were collected,and serum sCD146 level was detected by ELISA assay.Pearson method was used to analyze the correlation between the two indexes of breast cancer patients.Results For breast cancer patients,the level of serum sCD146 was higher than that in normal controls(P<0.05),while the methylation degree of MCAM gene in breast cancer tissues was significantly lower than that in adjacent tissues(P<0.05).The level of serum sCD146 was negatively correlated with the degree of MCAM gene methylation in tumor tissues(r=-0.461,P<0.001).In addition,the level of serum sCD146 and the degree of MCAM gene methylation in tumor tissues were correlated with T stage,grade,Ki-67 expression,ER and PR expression(P<0.05).The level of serum sCD146 and the low methylation rate of MCAM gene were the highest in the patients with triple negative breast cancer,which were significantly higher than those of Luminal A or Luminal B-HER2-patients(P<0.05).Conclusion There are relatively higher level of serum sCD146 in patients with breast cancer,especially in patients with triple negative breast cancer,which may be related to the demethylation of MCAM gene in tumor tissues.In addition,the level of serum sCD146 increases with the increase of tumor T stage and histological grade.
