基于CBCT对青少年下颌近中阻生第三磨牙分类特征的调查研究

摘　　要：目的:应用CBCT对青少年下颌近中阻生第三磨牙与邻牙或周围毗邻结构的位置关系进行分类,描述其分类特征。方法:收集行CBCT检查的400例山东地区青少年下颌近中阻生第三磨牙患者的影像学资料,对585颗下颌近中阻生第三磨牙进行三维重建分析,根据其与第二磨牙、下颌神经管及牙槽骨的关系,结合临床治疗需求进行统计分析。结果:①下颌近中阻生第三磨牙585颗中前倾、水平、倒逆、复合阻生构成比分别为50.43%(295/585)、48.89%(286/585)、0.34%(2/585)、0.34%(2/585)。②下颌前倾阻生第三磨牙高、中、低位阻生构成比分别为48.14%(142/295)、49.83%(147/295)、2.03%(6/295),水平阻生第三磨牙构成比分别为33.21%(95/286)、54.20%(155/286)、12.59%(36/286),复合阻生高、低位各1颗;倒逆阻生2颗均为低位阻生。③下颌近中阻生第三磨牙根尖与下颌神经管不接触、上壁接触、舌侧接触、颊侧接触、嵌入接触构成比分别为68.21%(399/585)、7.18%(42/585)、12.14%(71/585)、11.45%(67/585)、1.02%(6/585),接触率为31.79%(186/585)。④舌侧接触的下颌近中阻生第三磨牙根尖临舌侧骨板发生率为70.42%(50/71)。结论:基于CBCT的青少年下颌近中阻生第三磨牙可分为4个大类、9个亚类、60余个2级亚类,根尖与下颌神经管舌侧接触、颊侧接触率较高,与下颌神经管舌侧接触的根尖易临舌侧骨板。Objective:To classify the positional relationship between the mesial mandibular impacted third molars and the adjacent teeth or adjacent structures in adolescents by CBCT imaging,and to describe the classification characteristics.Methods:The imaging data of 400 patients with mesial mandibular impacted third molars in Shandong Province who underwent CBCT examination were collected.Three-dimensional reconstruction analysis was performed on 585 mesial impacted third molars of the mandible,and statistical descriptions were made based on their relationship with the second molars,the mandibular neural tube and the alveolar bone and the clinical treatment needs.Results:①The composition ratios of anterior,horizontal,inverted and compound impacted mesial impacted third molars of the mandible were 50.43%(295/585),48.89%(286/585),0.34%(2/585)and 0.34%(2/585),respectively.②The composition ratios of the high,medium and low impacted third molars of the mandibular anteversion were 48.14%(142/295),49.83%(147/295)and 2.03%(6/295),respectively.The composition ratios of the horizontal impacted third molars were 33.21%(95/286),54.20%(155/286)and 12.59%(36/286),respectively.There was 1 high and 1 low compound impacted teeth,and 2 inverted impacted molars were low impacted. ③The composition ratios of non-contact,superior wall contact,lingual contact,buccal contact and embedded contact between root tip and mandibular neural tube in the mesial mandibular impacted third molars were 68.21% (399/585),7.18% (42/585),12.14% (71/585),11.45% (67/585) and 1.02% (6/585). The contact rate was 31.79% (186/585). ④The incidence of root tip facingthe lingual bone plate of the mesial mandibular impacted third molars in contact with the lingual side of the mandibular nerve tube was 70.42% (50/71). Conclusions:Based on CBCT imaging,the mandibular impacted third molars of adolescents can be divided into 4 major categories,9 subcategories,and more than 60 secondary subcategories.
