摘 要:目的分析脑电图动态监测对脑出血后继发性癫痫的诊断价值,为脑电图动态监测的实践应用提供理论依据。方法52例脑出血后继发性癫痫的患者作为观察组,选择同期50例脑出血后未继发癫痫患者作为对照组。所有参与研究人员均给予脑电图动态监测。比较两组患者α、β、δ、θ各频段绝对功率以及脑电图慢波化比率(DTABR),统计两组患者动态脑电图癫痫样放电、非特异性异常放电的发生频率。结果观察组患者α、β、δ、θ频段绝对功率分别为(35.36±6.96)、(16.27±5.37)、(43.17±10.41)、(29.79±9.79)Hz,均高于对照组的(18.20±5.23)、(9.07±5.15)、(22.48±5.30)、(26.34±5.65)Hz,DTABR(0.84±0.33)低于对照组的(1.09±0.39),差异均有统计学意义(P<0.05)。观察组患者癫痫样放电发生率32.69%及非特异性异常放电发生率为42.31%均高于对照组的2.00%、10.00%,差异均有统计学意义(P<0.05)。结论脑电图动态监测对脑出血后继发性癫痫的患者具有重要的诊断价值,通过对患者脑电图各个频段功率的分析和异常放电状态的观察,能够及时的发现患者癫痫发病,为疾病的早期诊断早期治疗提供依据。Objective To analyze the diagnostic value of dynamic electroencephalogram monitoring for secondary epilepsy after intracerebral hemorrhage,and provide theoretical basis for the practical application of dynamic electroencephalogram monitoring.Methods There were 52 patients with secondary epilepsy after intracerebral hemorrhage served as the observation group,and 50 patients without secondary epilepsy after intracerebral hemorrhage during the same period as the control group.All participants received dynamic electroencephalogram monitoring.The absolute power in each of the frequency bandsα,β,δandθand deltatheta-alpha-beta ratio(DTABR)were compared between the two groups.The frequency of epileptiform discharges and non-specific abnormal discharges in dynamic electroencephalogram of the two groups were counted.Results The absolute power in each of the frequency bandsα,β,δandθof the observation group were(35.36±6.96),(16.27±5.37),(43.17±10.41)and(29.79±9.79)Hz,which were all higher than(18.20±5.23),(9.07±5.15),(22.48±5.30)and(26.34±5.65)Hz of the control group,and DTABR(0.84±0.33)was lower than(1.09±0.39)of the control group.All the difference was statistically significant(P<0.05).The incidence of epileptic discharge and non-specific abnormal discharge in the observation group were 32.69%and 42.31%,which were higher than 2.00%and 10.00%in the control group,and the difference was statistically significant(P<0.05).Conclusion Dynamic electroencephalogram monitoring has important diagnostic value for patients with secondary epilepsy after cerebral hemorrhage.Through the analysis of electroencephalogram power and observation of abnormal discharge state,we can find the onset of epilepsy of patients in time,and provide the basis for early diagnosis and treatment of the disease.
