摘　　要：目的探讨急诊经内镜逆行胰胆管造影术(ERCP)治疗急性化脓性胆管炎患者的临床效果。方法选取2014年3月至2019年3月本院收治的急性化脓性胆管炎患者70例作为研究对象,并根据治疗方案不同分为两组,各35例。对照组入院后进行积极抗感染等综合治疗,研究组在对照组基础上采用急诊ERCP治疗,比较两组治疗效果、死亡率、治疗前后胆红素水平变化。结果研究组治疗总有效率为97.1%,高于对照组的77.1%(P<0.05)。治疗后,研究组总胆红素、直接胆红素指标水平均低于对照组(P<0.05)。研究组死亡率低于对照组(P<0.05)。两组并发症发生率比较差异无统计学意义。结论急诊ERCP治疗急性化脓性胆管炎患者效果显著,可有效纠正感染性休克,降低胆红素水平,值得临床推广。Objective To explore the clinical effect of emergency endoscopic retrograde cholangiopancreatography(ERCP)in the treatment of acute suppurative cholangitis.Methods 70 patients with acute suppurative cholangitis diagnosed and treated in the hospital from March 2014 to March 2019 were selected as the research subjects,and they were divided into two groups according to different treatment regimens,with 35 cases in each group.The control group received active anti-infection and other comprehensive treatment after admission,and the study group received emergency ERCP treatment on the basis of the control group.The treatment effect,mortality,bilirubin before and after treatment were compared between the two groups.Results The total effective rate of the study group was 97.1%,which was higher than 77.1%of the control group(P<0.05);the levels of total bilirubin and direct bilirubin in the study group were lower than those in the control group(P<0.05);the mortality rate in the study group was lower than that in the control group(P<0.05);there was no significant difference in the incidence of complications between the two groups.Conclusion The clinical effect of emergency ERCP in the treatment of acute suppurative cholangitis is outstanding.It can effectively correct septic shock and reduce bilirubin level,which is worthy of clinical promotion.
