超声引导下聚桂醇硬化治疗子宫肌瘤临床疗效分析

摘　　要：目的观察超声引导下聚桂醇硬化治疗子宫肌瘤的疗效。方法2019年9月~2020年10月对唐山工人医院确诊为育龄期子宫肌瘤的30例患者(32个肌瘤)行超声引导下聚桂醇硬化治疗,于术前及术后1、2、3个月行常规超声、彩色多普勒超声及弹性评分测定检查,比较治疗前后肌瘤体积、血流分级及弹性评分的变化。结果术后1、2、3个月瘤体平均缩小率为(22.67±8.81)%、(36.41±11.38)%、(46.30±12.29)%,瘤体缩小显著;术后各时间点肌瘤血流较术前明显减少,差异均有统计学意义(P<0.001);术后肌瘤弹性评分较术前明显增加,术后3个月较1个月弹性评分明显增加,差异均有统计学意义(P<0.001);术中及术后不良反应轻微,无严重并发症发生。结论超声引导下聚桂醇硬化治疗子宫肌瘤疗效显著,不良反应轻微,具有较好的安全性和可行性,为围绝经期子宫肌瘤女性提供一种更加有效的治疗方法。Objective To observe the effect of ultrasound-guided sclerotherapy of uterine fibroids with lauromacrogol.Methods From September to October in 2019,30 patients(32 myomas)with uterine fibroids of childbearing age in Tangshan GongrenHospital were treated with ultrasound-guided lauryl alcohol sclerotherapy.The treatment of lauromacrogol was performed under the guidance of ultrasound.Routine ultrasound,color Doppler ultrasound and elastic score were performed before operation and 1,2 and 3 months after operation.The changes of leiomyoma volume,blood flow grading and elasticity score before and after treatment were compared.Results The mean tumor size reduction rate was(22.67±8.81)%,(36.41±11.38)% and(46.30±12.29)%at 1,2 and 3 months after operation(P<0.001);the blood flow of leiomyoma after operation was significantly decreased compared with that before operation(P<0.001);the hardness of myoma increased significantly after operation,and the hardness increased significantly after 3 months(P<0.001);the adverse reactions during and after operation were mild,and no serious complications occurred.Conclusion Ultrasound-guided sclerotherapy of uterine fibroids with lauromacrogol has significant curative effect,mild adverse reactions,and has good safety and feasibility.It provides a more effective treatment method for perimenopausal women with uterine fibroids to smoothly pass the perimenopausal period.
