虹膜环扎改良术治疗外伤性瞳孔散大的疗效观察

摘　　要：目的探讨虹膜环扎术的改良手术方式和传统手术方式的术后临床效果。方法2009年5月至2020年10月在四川省人民医院眼科接受虹膜环扎手术的患者28例(28只眼),其中18只眼采用虹膜环扎改良术,10只眼采用传统的虹膜环扎术。比较两种手术方式的术后效果。结果两种手术方式在术后视力及畏光感评分方面均有改善(P<0.05),但组间比较差异无统计学意义(P>0.05);改良术式组缝合针数及瞳孔最短径/最长径比值均大于传统组(P<0.01);针脚距瞳孔缘距离变异系数CV改良<CV传统。结论虹膜环扎改良术对外伤性瞳孔散大外形的改善比传统术式更稳定,对矫正视力及畏光感的改善与传统术式相比没有差异。Objective To compare the postoperative outcome of a new modified approach and traditional approach of iris cerclage.Methods Twenty-eight patients(28 eyes)were treated with iris cerclage from May 2009 to October 2020 in our hospital.Among the 28 eyes,18 were treated with modified operation and 10 were treated with traditional approach.The postoperative outcomes were compared between the two methods.Results The results showed an improvement in the Best Corrected Visual Acuity(BCVA)and photophobia index in both operation methods(P<0.05),but no significant difference was found between the two groups(P>0.05).There was significant difference in the number of stitches and in the value of shortest/longest diameter between the modified and traditional methods.The coefficient of variation(CV)of distance between stitch and pupil margin of the modified approach was smaller than that of the traditional approach.Conclusion The modified operation technique is more stable in pupil outlook for the treatment of traumatic tonic pupils than the traditional technique although there is no significant difference in corrected visual acuity and photophobia index between the two methods.
