三酰甘油与脑出血临床预后的相关性研究

摘　　要：目的探究三酰甘油水平与自发性脑出血临床预后之间的相关性。方法收集2005年5月-2018年5月在天津市环湖医院卒中单元病房住院的1451例自发性脑出血患者一般资料、血脂水平,随访3个月后记录改良Rankin量表(mRS)评分,根据mRS评分分为预后良好组和预后不良组。采用单因素及多因素Logistic回归分析两组患者三酰甘油水平与临床预后之间的相关性。结果共有1441例完成3个月的随访,随访率为99.31%。随访3个月后发现,预后良好组与预后不良组患者的女性占比、平均年龄、出血量、NHISS评分、三酰甘油和高密度脂蛋白水平比较,差异均有统计学意义(P<0.05)。单因素相关分析显示,平均年龄、NHISS评分、脑出血量、三酰甘油水平是脑出血后3个月预后不良的独立预测因子。多因素Logistic分析结果显示,平均年龄、三酰甘油水平、NHISS评分、脑出血量与脑出血后3个月临床预后不良独立相关。结论较低的三酰甘油水平是自发性脑出血患者不良短期预后的独立预测因子。Objective To explore the correlation between triglyceride level and clinical prognosis of spontaneous cerebral hemorrhage.Methods The general data and blood lipid levels of 1451 patients with spontaneous cerebral hemorrhage admitted to the Stroke Unit of Tianjin Huanhu Hospital from May 2005 to May 2018 were collected.The modified Rankin scale(m RS)score was recorded after 3 months of follow-up.According to m RS score,they were divided into good prognosis group and poor prognosis group.Univariate and multivariate Logistic regression were used to analyze the correlation between triglyceride level and clinical prognosis.Results A total of 1441 cases were followed-up for 3 months,and the follow-up rate was99.31%.After 3 months of follow-up,it was found that there were significant differences in female proportion,average age,blood loss,NHISS score,triglyceride and high-density lipoprotein levels between the two groups(P<0.05).Univariate correlation analysis showed that mean age,NHISS score,cerebral hemorrhage volume and triglyceride level were independent predictors of poor prognosis at 3 months after intracerebral hemorrhage.Multivariate Logistic analysis showed that mean age,triglyceride level,NHISS score and cerebral hemorrhage volume were independently associated with poor clinical prognosis 3 months after cerebral hemorrhage.Conclusion Low triglyceride level was an independent predictor of poor shortterm prognosis in patients with spontaneous intracerebral hemorrhage.
