胆囊切除术与结直肠癌相关性的研究现状

摘　　要：胆囊结石症是临床常见的一类外科疾病,胆囊切除术是治疗胆囊结石症最有效的方法。结直肠癌(CRC)是胃肠道中发生率较高的一类恶性肿瘤,目前研究发现,CRC发病率呈逐渐上升的趋势。CRC包括结肠癌和直肠癌,发病率从高到低依次为直肠、乙状结肠、盲肠、升结肠、降结肠及横结肠,近年来有向近端(右半结肠)发展的趋势,发病率和死亡率分别位于恶性肿瘤第3位及第4位,手术切除后的5年生存率平均可达40%~60%。早期发现、早期诊断、早期治疗以及开展规范化的手术治疗仍是提高CRC疗效的关键。临床许多研究学者对胆囊切除术与CRC的关系做了很多探讨,已开始从分子生物学层面着手研究CRC与胆囊疾病间的相关性,发现在胆囊切除后的患者CRC发病的风险增加。本文综述了胆囊切除术与CRC相关性的研究现状,以期为CRC的预防及治疗提供帮助。Cholecystolithiasis is a common clinical surgical disease, cholecystectomy is the most effective method for the treatment of cholecystolithiasis. Colorectal cancer(CRC) is a kind of malignant tumor with higher incidence rate in gastrointestinal tract. Currently, the incidence rate of CRC is increasing. CRC includes colon cancer and rectal cancer. The incidence rates from high to low are as follows: rectum,sigmoid colon, cecum, ascending colon, descending colon and transverse colon. In recent years, there is a trend towards the proximal(right colon) development. Incidence rate and mortality rate are third and fourth respectively in malignant tumors. The 5 years survival rate after operation is 40% to 60%. Early detection,early diagnosis, early treatment and standardized surgical treatment are still the key to improve the curative effect of CRC. Many clinical researchers have done a lot of research on the relationship between cholecystectomy and CRC. They have begun to study the correlation between CRC and gallbladder diseases from the molecular biology level, and found that the risk of CRC is increased in patients after cholecystectomy. This article reviews the research status of the correlation between cholecystectomy and CRC, in order to provide help for the prevention and treatment of CRC.
