摘  要:目的探究肠内免疫微生态营养支持对炎症性肠病(inflammatory bowel diseases,IBD)大鼠肠黏膜的保护作用及对辅助性T细胞17(T helper cell 17,Th17)细胞表达的影响。方法将40只SD幼鼠按照完全随机法分为对照组、模型组、肠内营养(enteral nutrition,EN)组和肠内免疫微生态营养(enteral ecoimmunonutrition,EIN)组,每组10只。对照组10只幼鼠仅进行常规饲养,其余30只SD幼鼠则采用三硝基苯磺酸(trinitro-benzene-sulfonic acid,TNBS)进行诱导,以构建IBD模型。模型组10只幼鼠给予0.9%生理盐水灌肠,EN组10只幼鼠给予常规肠内营养剂,EIN组10只幼鼠则在肠内营养基础上给予免疫微生态营养支持,观察四组幼鼠经不同干预后体质量、疾病活动指数评分(disease activity index,DAI)以及肠黏膜损伤情况。采用免疫组化法检测各组幼鼠肠黏膜组织切片中Th17细胞相关因子白细胞介素17A(interleukin 17A,IL-17A)的表达。结果与模型组相比,EN组和EIN组的肿瘤坏死因子-α(tumor necrosis factor alpha,TNF-α)、C-反应蛋白(C-reactive protein,CRP)水平显著下降,且EIN组水平更低(P<0.05),EN组和EIN组大鼠体质量增长值明显增加,且EIN组增长值更高(P<0.05)。EN组和EIN组DAI评分均显著低于模型组,且EIN组的评分更低(P<0.05)。与模型组相比,EN组和EIN组IL-17A阳性细胞染色数显著减少,且EIN组阳性细胞数更少(P<0.05)。结论肠内免疫微生态营养支持通过抑制IBD幼鼠Th17细胞相关因子IL-17A的表达,发挥保护肠黏膜的作用,显著改善IBD幼鼠肠道炎症反应。Objective To explore the protective effect of enteral ecoimmunonutrition support on the intestinal mucosa of rats with inflammatory bowel diseases(IBD)and its influence on the expression of T helper cell 17(Th17).Methods Forty SD infant rats were completely randomized into control group,model group,enteral nutrition(EN)group and enteral ecoimmunonutrition(EIN)group,with 10 rats in each group.Ten infant rats in control group were fed routinely,and the other 30 SD infant rats were induced with trinitro-benzene-sulfonic acid(TNBS)to construct an IBD model.Ten infant rats in model group were given 0.9%normal saline enema,ten infant rats in EN group were given conventional enteral nutrients,and ten infant rats in EIN group were given ecoimmunonutrition support on the basis of enteral nutrition.Body mass,disease activity index(DAI)and intestinal mucosal injury of rats after different interventions were observed and compared among the four groups.Immunohistochemistry was used to detect the expression of Th17 cell-related factor interleukin 17A(IL-17A)in the intestinal mucosal tissue sections of infant rats in each group.Results Compared with model group,the levels of tumor necrosis factorα(TNF-α)and C-reactive protein(CRP)were significantly decreased in EN group and EIN group,and the levels were lower in EIN group than in EN group(P<0.05).The body mass gain of rats was significantly higher in EN group and EIN group than in model group,and the gain was higher in EIN group than in EN group(P<0.05).DAI score was significantly lower in EN group and EIN group than in model group,and the score was lower in EIN group than in EN group(P<0.05).Compared with model group,the staining number of IL-17A positive cells was significantly reduced in EN group and EIN group,and the number of positive cells was less in EIN group than in EN group(P<0.05).Conclusion Enteral ecoimmunonutrition support can protect the intestinal mucosa by inhibiting the expression of Th17 cell-related factor IL-17A,and significantly improve the intestinal in
