低浓度阿托品协同夜戴角膜塑形镜治疗青少年近视研究

摘　　要：为了观察低浓度阿托品协同夜戴角膜塑形镜治疗青少年中、低度近视的效果及不良反应,随机选择淮南市8~14岁患者164例325眼,随机分为3组,即0.01%阿托品联合夜戴角膜塑形镜组53例105眼(协同治疗组),夜戴角膜塑形镜组(单独治疗组)56例110眼;普通框架镜组(对照组)55例110眼。正确配戴镜片及使用本院配制的0.01%阿托品滴眼液进行治疗,在12月、24月时复查,前两组需停戴塑形镜和药物4周。观察裸眼视力、屈光度、眼轴等各项指标。结果显示,治疗12月时,协同治疗组治疗效果仅优于对照组,治疗24月时,协同治疗组主要观察指标均优于单独治疗组和对照组;协同治疗组12月、24月时角膜点染、畏光、眼痛等均略多于单独治疗组,但无严重并发症。因此,低浓度阿托品协同夜戴角膜塑形镜在长期(至少大于1 a)控制中低度近视是安全、有效的。To observe the effect and adverse reactions of anticholinergic drugs combined with night orthokeratology in the treatment of low and moderate myopia in adolescents.Totally 164 adolescents with moderate to low myopia(325 eyes)aged 8-14 years were randomly selected from Huainan City.They were randomly divided into three groups:the 0.01%atropine combined with orthokeratology group(53 cases,105 eyes),the orthokeratology group(56 cases,110 eyes)and the control group(55 cases,110 eyes).The three groups of adolescents were treated with conventional corrective wearing lenses and medication respectively.All the patients in the three groups were reexamined at 12 months and 24 months.The first two groups needed to stop wearing orthokeratology and drugs for 4 weeks.The uncorrected visual acuity,diopter and axial length were observed.The results showed that the therapeutic effect of the cooperative treatment group was only better than that of the control group at 12 m,and the main observation indexes of the cooperative treatment group were better than those of the single treatment group and the control group at 24 m;the corneal staining,photophobia and eye pain of the cooperative treatment group were slightly more than those of the single treatment group at 12 m and 24 m,but there were no serious complications.Therefore,low atropine combined with night wear orthokeratology is safe and effective in controlling low and moderate myopia for a long time(at least more than 1a).
