扶正祛瘀法治疗帕金森病有效性的Meta分析

摘　　要：目的系统评价扶正祛瘀法治疗帕金森病(PD)的临床疗效。方法计算机检索中国期刊全文数据库(CNKI)、万方资源数据库、维普资源整合平台(VIP)、中国生物医学文献网络版数据库(SinoMed)、PubMed、The Cochrane Library、EMbase等数据库中采用扶正祛瘀法治疗帕金森病的临床研究文献,检索时限为建库至2020年12月31日,由两名研究人员根据纳入标准、排除标准进行文献纳入并提取数据,评价文献质量,采用Cochrane协作网提供的RevMan 5.4软件进行Meta分析。结果纳入30项研究,涉及2437例病人,其中治疗组1241例,对照组1196例。Meta分析结果显示,治疗组临床疗效总有效率高于对照组[RR=1.38,95%CI(1.21,1.57),P<0.00001];治疗组治疗后帕金森病综合评分量表(UPDRS)评分[MD=-4.94,95%CI(-7.42,-2.46),P<0.0001]、UPDRSⅠ评分[MD=-0.57,95%CI(-1.10,-0.05),P<0.05]、UPDRSⅡ评分[MD=-2.70,95%CI(-3.98,-1.42),P<0.0001]、UPDRSⅢ评分[MD=-3.54,95%CI(-5.98,-1.10),P=0.004]、UPDRSⅣ评分[MD=-0.96,95%CI(-1.59,-0.33),P=0.003]、帕金森病人生活质量问卷(PDQ-39)评分[MD=-5.63,95%CI(-9.42,-1.83),P=0.004]均低于对照组,差异均有统计学意义。结论现有证据表明:扶正祛瘀法治疗帕金森病可以改善病人临床症状,提高病人生活质量。Objective To evaluate the efficacy of Fuzheng Quyu method for the treatment of Parkinson′s disease.Methods CNKI,Wanfang,VIP,SinoMed,PubMed,EMbase,and The Cochrane Library were searched for clinical literature on Fuzheng Quyu Method for the treatment of Parkinson′s disease from the establishment of the database to December 31,2020.Based on criteria of inclusion and exclusion,two researchers screened literature,extracted data,assessed quality independently and analyzed results by RevMan5.4 software.Results Thirty studies involving 2437 patients were included,including 1241 cases in the Fuzheng Quyu treatment group and 1196 cases in the control group.The Meta-analysis results showed that the total effective rate of Fuzheng Quyu treatment group was significantly higher than that of control group[RR=1.38,95%CI(1.21,1.57),P<0.00001],the scores of Unified Parkinson′s Disease Rating Scale(UPDRS)[MD=-4.94,95%CI(-7.42,-2.46),P<0.0001],UPDRSⅠ[MD=-0.57,95%CI(-1.10,-0.05),P<0.05],UPDRSⅡ[MD=-2.70,95%CI(-3.98,-1.42),P<0.0001],UPDRSⅢ[MD=-3.54,95%CI(-5.98,-1.10),P=0.004],UPDRSⅣ[MD=-0.96,95%CI(-1.59,-0.33),P=0.003],PDQ-39[MD=-5.63,95%CI(-9.42,-1.83),P=0.004]of Fuzheng Quyu treatment group were lower than that of control group.Conclusion The available evidence suggests that Fuzheng Quyu method for the treatment of Parkinson′s disease can improve the clinical symptoms,improve the quality of life of patients.
