玻璃体切除术前玻璃体内注射雷珠单抗治疗新生血管性青光眼

摘　　要：目的:观察玻璃体内注射雷珠单抗然后进行玻璃体切除术及广泛视网膜光凝治疗新生血管性青光眼的临床效果。方法:收集焦作市人民医院2017年10月至2019年10月收治的新生血管性青光眼60例(60眼)的临床资料,进行回顾性研究。根据治疗方式不同将患者分为治疗组和对照组。治疗组,30例,先行玻璃体内注射雷珠单抗,5 d后行玻璃体切除术,术中进行广泛视网膜光凝。对照组,30例,行睫状前动脉结扎联合睫状体冷凝术。术后随访2个月,观察两组眼压、视力及临床症状。结果:两组术后1周、1个月及2个月眼压均低于术前,且治疗组明显低于对照组( t=4.795, 5.176, 5.101;均 P<0.01)。治疗组术后视力明显优于对照组,临床症状缓解更加明显。 结论:玻璃体内注射雷珠单抗后进行玻璃体切除术及广泛视网膜光凝术治疗新生血管性青光眼安全有效。Objective To observe the clinical efficacy of intravitreal injection of ranibizumab after vitrectomy and panretinal photocoagulation for the treatment of neovascular glaucoma.Methods The clinical data of 60 eyes of 60 cases of neovasclar glaucoma from Oct.2017 to Oct.2019 in the People’s Hospital of Jiaozuo were collected and analyzed retrospectively.According to the different treatment methods,all cases were divided into treatment group and control group.Total of 30 cases in the treatment group were treated with vitrectomy and panretinal photocoagulation at 5 days after intravitreal injection of ranibizumab.Total of 30 cases in the control group received anterior ciliary artery ligation combined with ciliary body condensation.The follow-up time was 2 months after operation.The intraocular pressure,visual acuity and clinical symptom were observed.Results The postoperative intraocular pressure of the two groups were lower than preoperative intraocular pressure,and the postoperative intraocular pressure of the treatment group was significantly lower than that of the control group(t=4.795,5.176,5.101;and P<0.001).The postoperative visual acuity of the treatment group was significantly better than that of the control group,and the clinical symptoms were relieved obviously.Conclusion Intravitreal injection of ranibizumab before vitrectomy for the treatment of neovascular glaucoma is safe and effective.
