摘　　要：目的:探讨急性等容性血液稀释在腓骨带蒂肌皮瓣修复口腔癌术后缺损中的有效性和安全性。方法:回顾分析行口腔癌根治及游离腓骨带蒂肌皮瓣修复手术的患者62例,根据术前是否采集自体血分为ANH组(n=31)及对照组(n=31),比较两组患者术中血流动力学(MAP、HR、SVV、CI)的变化,术中异体血输注量及术后皮瓣相关并发症的差异。结果:与对照组相比,ANH组患者异体血输注量明显减少(P<0.05),两组患者术中血流动力学变化及术后皮瓣相关并发症无明显差异。结论:急性等容性血液稀释可有效的减少腓骨带蒂肌皮瓣移植手术患者围手术期同型异体输血。Objective:To evaluate the effectiveness and safety of acute normovolemic hemodilution(ANH)inpatients undergoingfibula osseo-myocutaneous flap reconstruction of the oral cancer. Methods:A retrospective analysis of62 patients who underwent oral cancer radical surgery with fibula osseo-myocutaneous flap reconstruction were divided into ANH group(n=31)and control group(n=31)according to whether autologous blood was collected before operation or not. Differences were observed in intraoperative hemodynamics(MAP,HR,SVV,CI),intraoperative allogeneic blood transfusion and postoperative flap-related complications between the two groups. Results:Compared with the control group,the amount of allogeneic blood transfusion was significantly reduced in the ANH group(P<0.05). There was no significant difference in intraoperative hemodynamic changes and postoperative flap-related complications between the two groups. Conclusion:Acute normovolemic hemodilution can effectively reduce perioperative homologous blood transfusion in patients undergoing fibula osseo-myocutaneous flap reconstruction of the oral cancer without increased complications.
