LAVH和LTH两种手术方式在巨大子宫肌瘤中的应用对比分析

摘　　要：目的:探讨腹腔镜辅助阴式全子宫切除术(LAVH)和腹腔镜全子宫切除术(LTH)在巨大子宫肌瘤中的治疗效果。方法:选取2018年6月-2019年10月于本院行腹腔镜子宫切除手术的198例巨大子宫肌瘤患者为研究对象,采用随机数字表分法将患者分为LTH组与LAVH组,每组99例。LTH组给予LTH治疗,LAVH组给予LAVH治疗。比较两组手术相关指标及并发症发生情况。结果:LAVH组手术时间、术后留置尿管时间、术后首次排气时间及术后首次下床时间均长于LTH组,术中出血量高于LTH组,而手术费用低于LTH组,差异均有统计学意义(P<0.05)。两组住院时间比较,差异无统计学意义(P>0.05)。LTH组并发症发生率(39.39%)高于LAVH组(7.07%),差异有统计学意义(P<0.05)。结论:与LTH相比,LAVH治疗巨大子宫肌瘤患者可降低并发症发生风险,且手术费用较低。Objective:To investigate the effect of laparoscopic assisted vaginal hysterectomy(LAVH)and laparoscopic total hysterectomy(LTH)in the treatment of giant uterine fibroids.Method:A total of 198 patients with giant uterine fibroids who underwent laparoscopic hysterectomy in our hospital from June 2018 to October 2019 were selected as the research subjects.The patients were divided into LTH group and LAVH group by random number table partition method,with 99 patients in each group.LTH treatment was given to the LTH group and LAVH treatment was given to the LAVH group.Operation-related indexes and complications were compared between the two groups.Result:The operative time,postoperative indwelling catheter time,postoperative first exhaust time and postoperative first get out of bed time in LAVH group were longer than those in LTH group,the amount of intraoperative blood loss was higher than that in LTH group,but the operative cost was lower than that in LTH group,the differences were statistically significant(P<0.05).There was no significant difference in the length of stay between the two groups(P>0.05).The incidence of complications in LTH group(39.39%)was higher than that in LAVH group(7.07%),and the difference was statistically significant(P<0.05).Conclusion:Compared with LTH,LAVH treatment of patients with giant uterine fibroids reduces the risk of complications and has lower surgical costs.
