超声乳化术治疗后极性白内障临床效果观察

摘　　要：目的探讨白内障超声乳化摘除术治疗后极性白内障临床效果。方法回顾性分析我院2017年1月至2019年11月共收治21例(32眼)后极性白内障患者,由同一术者行白内障超声乳化摘除联合人工晶体植入术,观察术中后囊膜破裂发生率,术后最佳矫正视力及术后并发症情况。同时与我院同期收治的由同一手术医生完成的普通白内障2047眼后囊膜破裂发生率进行对比。结果21例(32眼)后极性白内障患者中,后囊完整透明19眼(占59.4%),术中后囊膜破裂5眼(占15.6%),与同期普通白内障超声乳化术后后囊膜破裂发生率(0.9%)相比,差异有统计学意义(P<0.05)。术后3个月最佳矫正视力与术前相比均有不同程度提高。术后4眼发生明显角膜水肿,其中2眼合并术后眼压偏高,药物治疗后2天眼压恢复正常,1周后角膜透明。结论后极性白内障后囊破裂的发生率较高,但通过提高手术技巧及合理超声乳化参数设置,也能减少术中术后并发症,恢复良好视力。Objective To investigate the clinical effect of Phacoemulsification for Posterior polar cataract.Methods Retrospective analysis about A total of 21 patients(32 eyes)with posterior polar cataract were admitted to our hospital from January 2017 to November 2019.All phacoemulsification was performed by the same surgeon.To observe the incidence of posterior capsular rupture,the best corrected visual acuity and postoperative complications.At the same time,the incidence of posterior capsular rupture in 2047 common cataract eyes treated by the same surgeon in our hospital during the same period was compared.Results Of the 21 cases(32 eyes)with posterior polar cataract,the posterior capsule was intact and transparent in 19(59.4%)and the posterior capsule was ruptured in 5(15.6%).Compared with the incidence of posterior capsular rupture after phacoemulsification(0.9%)during the same period,the difference was statistically significant(P<0.05).The best corrected visual acuity was improved at 3 months after operation compared with that before operation.Postoperative corneal edema was observed in 4 eyes,and intraocular pressure was higher in 2 eyes.Conclusions The incidence of posterior capsular rupture in posterior polar cataract is higher,but by improving surgical skills and setting reasonable phacoemulsification parameters,intraoperative and postoperative complications can be reduced and good visual acuity can be restored.
