聚桂醇联合组织胶应用于食管胃底静脉曲张破裂出血的内镜治疗时机的研究

摘　　要：目的:探讨聚桂醇联合组织胶在食管胃底静脉曲张破裂出血(EGVB)的内镜治疗时机。方法:统计急诊内镜止血应用前后EGVB死亡率,回顾分析2014年-2019年于我院行食管胃底静脉曲张聚桂醇联合组织胶治疗患者167例,其中急诊治疗组(A组,入院后12小时内)39例,非急诊治疗组(B组,入院后12小时以上)128例,比较两组患者住院费用及时间,术后再出血率,不良反应发生率。结果:急诊内镜止血应用前EGVB死亡率(21.9%)高于应用后(9.1%),差异有统计学意义(P<0.05);A组患者住院费用(25063.78±4365.12)元,高于B组(19658.57±3274.08)元,差异有统计学意义(P<0.05);A组患者住院时间(8.5±1.8)天,高于B组(6.5±1.2天),差异有统计学意义(P<0.05);A组术后再出血率(12.8%)略高于B组(10.9%),差异无统计学意义(P>0.05);A组不良反应发生率(30.7%)略高于B组(27.3%),差异无统计学意义(P>0.05)。结论:聚桂醇联合组织胶在EGVB急诊止血中有着良好的应用效果,EGVB患者在血流动力学稳定的情况下,应尽早行内镜治疗。Objective:To investigate the timing of endoscopic treatment of cinnamyl alcohol combined with tissue glue in the treatment of esophageal gastric varices bleeding(EGVB).Methods:Calculate the mortality of EGVB before and after the application of emergency endoscopic hemostasis,A retrospective analysis of 167 patients undergoing esophagogastric varices combined with tissue glue in our hospital from 2014 to 2019,including 39 cases in the emergency treatment group(group A,Within 12 hours after hospitalization)and 128 cases in the non-emergency treatment group(group B,More than 12 hours after hospitalization),compared Hospitalization cost and time of two groups of patients,postoperative rebleeding rate,incidence of adverse reactions.Results:The mortality rate of EGVB before the application of emergency endoscopic hemostasis(21.9%)was higher than that after the application(9.1%),the difference was statistically significant(P<0.05),The hospitalization cost of patients in group A(25063.78±4365.12)yuan was higher than that in group B(19658.57±3274.08)yuan,the difference was statistically significant(P<0.05).The hospital stay of group A(8.5±1.8)days was higher than that of group B(6.5±1.2)days,the difference was statistically significant(P<0.05).Postoperative rebleeding rate in group A(12.8%)was slightly higher than that in group B(10.9%),the difference was not statistically significant(P>0.05).The incidence of adverse reactions in group A(30.7%)was slightly higher than that in group B(27.3%),the difference was not statistically significant(P>0.05).Conclusion:Polycinnamyl alcohol combined with tissue glue has a good application effect in EGVB emergency hemostasis,patients with EGVB should undergo endoscopic treatment as soon as possible when the hemodynamics are stable.
