含铋剂四联疗法对HP阳性胃溃疡患者临床症状及炎性因子水平的影响

摘　　要：目的研究含铋剂四联疗法对幽门螺旋杆菌(HP)阳性胃溃疡患者临床症状及炎性因子水平的影响。方法研究对象为2019年1月—2020年1月入院的76例HP阳性胃溃疡患者,参考随机数字表法分为对照组与研究组,各38例;对照组选用甲硝唑片、多西环素、兰索拉唑肠溶片三联疗法治疗,研究组选用甲硝唑片、多西环素、兰索拉唑肠溶片、胶体酒石酸铋胶囊四联疗法;比较两组治疗前后临床症状评分、炎性因子(C-反应蛋白、白介素-10、白介素-6)水平变化与治疗后总有效率。结果经治疗后,两组较治疗前临床症状评分、炎性因子水平更低,差异有统计学意义(P<0.05);研究组临床症状评分(3.34±1.27)分、C-反应蛋白(3.09±0.62)mg·dl-1、白介素-10(56.32±6.65)ng·mL^(-1)、白介素-6(21.64±3.62)pg·mL^(-1)均明显低于对照组,两组比较差异有统计学意义(t=19.178、5.722、12.357,P<0.05);研究组总有效率(97.37%)高于对照组(78.95%),差异有统计学意义(χ^(2)=4.537,P<0.05)。结论含铋剂四联疗法治疗HP阳性胃溃疡患者的效果显著,可有效改善患者临床症状,降低炎性因子水平。Objective To study the effect of bismuth-containing quadruple therapy on the clinical symptoms and levels of inflammatory factors in patients with Helicobacter pylori(HP)positive gastric ulcer.Methods The subjects of the study were 76 HP-positive gastric ulcer patients admitted to the hospital from January 2019 to January 2020.They were divided into two groups with reference to the random number table method,namely the control group and the study group,with 38 cases in each group;the control group adopted the triple therapy of metronidazole tablets,doxycycline,and lansoprazole enteric-coated tablets,and the research group used metronidazole tablets,doxycycline,lansoprazole enteric-coated tablets,and colloidal bismuth tartrate capsules quadruple therapy;Comparison of clinical symptom scores,levels of inflammatory factors(C-reactive protein,interleukin-10,and interleukin-6)before and after treatment between the two groups and the total effective rate after treatment.Results After treatment,the two groups had lower clinical symptom scores and inflammatory factor levels than before treatment,the difference was statistically significant(P<0.05);the clinical symptom scores of the study group(3.34±1.27)points,C-reactive protein(3.09±0.62)mg·dl-1.Interleukin-10(56.32±6.65)ng·mL^(-1)and interleukin-6(21.64±3.62)pg·mL^(-1)were significantly lower than those of the control group,and the difference between the two groups was statistically significant(t=19.178,5.722,12.357,P<0.05);the total effective rate of the study group(97.37%)was higher than that of the control group(78.95%),the difference was statistically significant(χ^(2)=4.537,P<0.05).Conclusion The bismuth-containing quadruple therapy is effective in treating patients with HP-positive gastric ulcer,which can effectively improve the clinical symptoms of patients and reduce the level of inflammatory factors.
