摘  要:目的观察视觉训练联合防周边远视性光学离焦镜片控制青少年低、中度近视的临床疗效。方法选取2018年3月—2018年12月于唐山市眼科医院验光配镜中心确诊的近视患者270人(540只眼),随机分为常规组90例(180只眼)、功能组90例(180只眼)和联合组90例(180只眼)。常规组予单焦点球面镜片(框架眼镜)治疗,功能组予防周边远视性光学离焦镜片(框架眼镜)治疗,联合组在功能组的基础上联合视觉训练治疗。每组均观察12个月,并于治疗前和治疗后测量患者的屈光度和眼轴。结果(1)屈光度:3组治疗后均比治疗前升高,差异均有统计学意义(t_(常规组)=75.168,t_(功能组)=68.062,t_(联合组)=58.309,均P=0.000)。治疗后3组比较,与常规组比较,功能组和联合组均降低,差异均有统计学意义(t_(功能组)=69.972,t_(联合组)=67.063,均P=0.000)。与功能组比较,联合组降低,差异有统计学意义(t=46.650,P=0.000)。(2)眼轴:3组治疗后均比治疗前增长,差异均有统计学意义(t_(常规组)=39.650,t_(功能组)=71.139,t_(联合组)=31.453,均P=0.000)。治疗后3组比较,与常规组比较,功能组和联合组均缩短,差异均有统计学意义(t_(功能组)=35.242,t_(联合组)=61.671,均P=0.000)。与功能组比较,联合组缩短,差异有统计学意义(t=26.430,P=0.000)。结论视觉训练联合防周边远视性光学离焦镜片治疗可延缓青少年低中度近视的屈光度及眼轴的增长速度,临床效果优于单纯使用单焦点球面镜片及防周边远视性光学离焦镜片治疗。OBJECTIVE To observe the clinical effect of visual training combined with anti-peripheral hyperopia optical defocus lens on the control of low,moderate teenager myopia.METHODS A total of 270 myopia patients(540 eyes)diagnosed in the Optometry Center of Tangshan Eye Hospital from March 2018 to December 2018 were randomly divided into routine group(RG)with 90 cases(180 eyes),functional group(FG)with 90 cases(180 eyes)and combined group(CG)with 90 cases(180 eyes).The routine group was treated with Mingyue(brand name)single focus spherical lens(frame glasses).The functional group was treated with anti-hyperopia optical defocusing lens(frame glasses).And the combined group was treated with visual training on the basis of the functional group.Each group was observed for 12 months,and the diopter and ocular axis were measured before and after treatment.RESULTS(1)Diopter:After treatment,the three groups were all higher than before and the differences were all statistically significant(tRG=75.168,t_(FG)=68.062,t_(CG)=58.309,all P=0.000).After treatment,comparison with the conventional group showed that both the functional group and the combined group were lower,the differences were statistically significant(t_(FG)=69.972,t_(CG)=67.063,all P=0.000).Compared with the functional group,the combined group was significantly lower(t=46.650,P=0.000).(2)Eye axis:After treatment,results of the three groups all increased,and the differences were statistically significant(tRG=39.650,t_(FG)=71.139,t_(CG)=31.453,all P=0.000).After treatment,compared with the conventional group,both the functional group and the combined group were shorter,the differences were statistically significant(t_(FG)=35.242,t_(CG)=61.671,all P=0.000).Compared with the functional group,result of the combined group was shortened,and the difference was statistically significant(t=26.430,P=0.000).CONCLUSIONS Visual training combined with anti-peripheral hyperopia optical defocus lens can delay the rate of refraction and axial growth in teenager myopia,and the clinic
