摘 要:目的分析缺血性脑卒中患者肠道菌群及血清白细胞介素-6水平的变化。方法抽取2019年7月至2020年7月焦作煤业(集团)有限责任公司中央医院诊治的缺血性脑卒中患者50例纳入研究组,另抽取同期行常规体检的健康志愿者50例纳入对照组,两组均行肠道菌群(双歧杆菌、乳酸杆菌、肠球菌、奇异变形杆菌、艰难梭菌、链球菌、葡萄球菌)及血清白细胞介素-6水平检测,比较两组受试者的检测结果。应用美国国立卫生研究院卒中量表(NIHSS)评估缺血性脑卒中患者的神经功能,应用Pearson相关性分析法分析肠道菌群及血清白细胞介素-6水平与缺血性脑卒中患者NIHSS评分的相关性。结果研究组双歧杆菌、乳酸杆菌、奇异变形杆菌的含量均低于对照组,肠球菌、艰难梭菌、链球菌、葡萄球菌的含量以及血清白细胞介素-6水平高于对照组(P<0.05);Pearson相关性分析结果显示,双歧杆菌、乳酸杆菌、奇异变形杆菌与缺血性脑卒中患者的NIHSS评分呈负相关(r=-0.214、-0.177、-0.225,P<0.05),肠球菌、艰难梭菌、链球菌、葡萄球菌、血清白细胞介素-6水平与缺血性脑卒中患者的NIHSS评分均呈正相关性(r=-0.364、0.192、0.281、0.370、0.281,P均<0.05)。结论缺血性脑卒中患者肠道菌群含量和血清白细胞介素-6水平与健康志愿者相比存在异常变化,两项指标的检测可为临床诊断该病提供辅助参考依据。Objective To analyze the changes of intestinal flora and serum interleukin-6 levels in patients with ischemic stroke.Methods A total of 50 cases of ischemic stroke patients in Central Hospital of Jiaozuo Coal Industry Group from July 2019 to July 2020 were selected as the study group.Another 50 healthy volunteers who underwent routine physical examination at the same time in the hospital were included in the control group.The intestinal flora(Bifidobacterium,Lactobacillus,Enterococcus,Proteus mirabilis,Clostridium difficile,Streptococcus,Staphylococcus)and serum interleukin-6 levels were detected in the two groups.And the detection results of the two groups were compared.Nationd Institutes of Health stroke Scale(NIHSS)was used to evaluate the neurological function of patients with ischemic stroke.Pearson correlation analysis was used to analyze the correlation of intestinal flora and serum interleukin-6 levels with NIHSS score of patients with ischemic stroke.Results The contents of Bifidobacterium,Lactobacillus and Proteus mirabilis in the study group were lower than those in the control group,while the contents of Enterococcus,Clostridium difficile,Streptococcus,Staphylococcus and serum interleukin-6 levels in the study group were higher than those in the control group(P<0.05).Pearson correlation analysis showed that Bifidobacterium,Lactobacillus,Proteus mirabilis were negatively correlatied of patients with is chemic stroke with NIHSS scores(r=-0.214,-0.177,-0.225;all P<0.05);Enterococcus,Clostridium difficile Streptococcus,Staphylococcus,serum interleukin-6 levels were positively correlatied with NIHSS score of patients with ischemic stroke(r=-0.364,0.192,0.281,0.370,0.281;all P<0.05).Conclusions There are obvious abnormal changes in intestinal flora and serum interleukin-6 level in patients with ischemic stroke,compared with healthy volunteers.The detection of the two indexes can provide reliable reference for clinical diagnosis of the disease.
